You can review the Clinical Practice Guidelines for Management of Anal Fissure. It outlines the steps CRS should take in the treatment of AF. 6-12 weeks of conservative treatment with creams is pretty normal. If no response they should look at botox and then finally LIS. Surgery should really only be an option for chronic anal fissure patients.
If you're having trouble applying with a finger, try using an applicator to apply in the anal canal. Make sure you're using consistently 2-3x daily for the full 4-6 week regimen, even when you're not in pain.

1 - i would suggest getting a heat pad you can sit on or a hot water bottle to place next to your tail bone to help with the pain.. this will deffo help!!
2 - use coconut oil suppositories 5-10 mins before a BM and then one after, one before bed (you can make them yourself, get a suppository mold and only use raw virgin coconut oil.. no processed nonsense!)
3 - sitz bath with a drop of tea tree oil a couple of times a day, obviously don't overdo this.
4 - WASH your backside after a BM with clean water warm.... use an empty bottle if you don't have a bidet
5 - diet.... make sure you're having enough fibre and water during the day.
6 - i would also suggest trying these along with a course of antibiotics as they helped bring my chronic fissure pain down to almost nothing.... (i've had a fissure since january!)
7 - long daily walks... it deffo helps with getting blood to that area.

I'm so glad it's helping! The homeopathic doctor I consulted also said to take 4 of these pills before each meal. I didn't take it because the ointment helped on its own but could be helpful!
I just wanted to say that I have now tried this Calendu stuff for a few days and it's been wonderful.
It does sting and burn me upon initial application, but that disappates in a few minutes. It then really starts doing its magic. I really recommend trying this.

I find most bidets to be to harsh of a water stream, which can actually be the cause of a fissure in the first place.
I currently utilize a hand-held hose which acts like a bidet, but I can control the water stream to lightly 'spritz' to clean most post BM. Biodegradable baby wipes are then used to clean after the hose.

Can I suggest a portable bidet? I bought one just for that reason. So far so good. Here's the one I got:
I use lotion injector. It's like your pinky size and the lotion goes in very clean and completely. With this you can do it yourself without the help of anyone. Please try this, it helped me inserting lotion completely without wasting.

Glad to hear that this might be helpful for you! I have one bowel movement a day always in the morning on 90% of the days. On the other days I have 2-3 bowel movements.
For me its the same: too hard is not good as this risks a retear and too soft is also not good as there will be leftover fecal matter. The first is addressed by taking Movicol each evening and the second by cleaning inside with that "fissure pen".
The itching really started before I got my first fissure and was always somehow connected to the fissure. Therefore I think it could definitely be the cause.
I live in the EU and the fissure pen can be bought here via Amazon. Therefore I think you could even order it abroad with a little bit more shipping cost or directly from their online shop. They also offer cleaning gels and oils which are pretty useless I think. For me it works perfectly fine only using water based lube.

Sometimes it's hard to tell the difference, I've suffered from fissures and hemorrhoids and experienced burning sensations with both. The only real tip off I can think of for a fissure is that it distinctly hurts/burn while you're actually passing a BM. My hemorrhoids tend to be bloodier than my fissures, but don't hurt while passing a BM. More severe hemorrhoids maybe different though, I'm not sure. Regarding wiping I highly recommend a bidet, makes it more pleasant and less painful. I'm currently too cheap to set up my toilet with one but this product I'll link below is very cheap and nice and I take it with me traveling. Calmoseptine may help with itch and burn. Or you may absolute hate it, it has menthol which can be a bit intense. A little goes a very long way, start small if you decide to try it, and maybe put a layer of coconut or olive oil on first. I think they mean a peri-bottle, it’s just a bottle with a long neck that is bent at a sharp angle that when you turn it upside down and squeeze the bottle, water comes out. I’ve been using this one to clean after a BM but this is a great suggestion to use it for its intended use haha

In the end, the list of things that helped heal me was keeping the area very clean (bought a bidet toilet seat), sitz baths using the contraption that sits over the toilet, never straining to have a BM (i had a high fiber smoothie daily and took magnesium), drinking tons of water, icing, and a niphedipine prescription from my doctor which I applied religiously for 8 + weeks. I never had headaches from niphedipine. I have been healed for almost a year now!!
I sing the praises of DoseRite every chance I get. It's basically a syringe with an applicator tip that you insert (you can add lubrication to the applicator for even easier insertion). The tip is very thin, I have very small and bony fingers and it's smaller than my pinky. MUCH easier and far more precise and sanitary than fingers. You can find it on Amazon.
Just a couple of other suggestions to ease the discomfort. I bought a seat that fits on my toilet and you can fill it up with warm water and make a sitz bath. I used these drops in it as well. After each BM, I would get into shower and use water to clean. Then I would sit on my seat for 15 minutes.
I would also moisturize after because that seemed to help. I bought calmoseptine ointment, which honestly helped only some days. Coconut oil, Vaseline worked just as well. Also good to be moisturized before a BM.
Heating pads were helpful sometimes.
I haven’t had a painful BM in months, though I still have one or two fissures. I take a capful Miralax every day and drink an excessive amount of water.

Not OP, but I've also been using butt plugs recently and found them really helpful. As far as I'm aware, it's a myth that butt plugs can give you hemorrhoids. You're more likely to get one from straining too hard when going to the toilet.
I bought a set similar to this and there are lots of other places that sell them. I would start with the smallest one, and a decent amount of some sort of lubricant (I use either water-based lube or Vaseline). You shouldn't push it in hard or anything, I usually just apply a bit of pressure with it on my anus and it will slowly start to go in. The first couple of times I used it I had a bit of pain, but it soon went away once it was in. I hope this helps :)

The difference between applying the cream externally vs internally affects how the medication is absorbed in your body. Applying the cream inside the anal canal will lead to local absorption by the internal sphincter muscle- the ideal target. When you apply externally you are relying on systemic absorption- absorption through the entire bloodstream. The systemic approach is going to be more diluted- a shotgun versus a rifle in terms of targeting a specific area. Systemic absorption is also thought to cause more headaches. Take a look at this paper, which shows local absorption (inside) was more effective reducing anal resting pressure than systemic absorption (outside).
To apply inside use an anal applicator or a gloved finger in order to minimize headaches. Alternatively you could use a calcium channel blocker (Nifedipine), which has a better side effect profile (i.e. fewer headaches). Nifedipine also needs to be applied locally- inside the anal canal.

It doesn’t sound any different than taking a regular bath but it helps more because you’re elevated in the water. Helps with healing and just feels soothing after a bad flare up. You can find them on amazon or at your local pharmacy - I have this one! I got the same one at my pharmacy for about $14.

Hi there Ive actually had a ton of success with dilation. As for what kit to get, i just got this one on amazon for cheap and it has worked well for me. I started with a finger and moved up slowly to the smallest one and so on. The key is to go slow and try to work at it daily. One thing thats also helped make the dilation easier is pelvic floor stretching. It helps loosen those big muscles around your anus making dilation a little easier. You can look up any number of videos on stretches for pelvic floor release. A guy on youtube called uptown mike was helpful. Ive been at this for about a month and a half and since then i have not bled at all, still minor discomfort sometimes but after a year of chronic fissures, i know they are not healing over night. If you want some more detail check out the post i made about a week ago. Hopefully this helps you a little bit. I totally understand the frustration that comes with nothing working time after time.
The nifedipine has helped. Honestly, just time. It used to hurt for hours after a BM and now it’s minutes. Sometime a sitz bath helps and sometimes it doesn’t.
