I have no idea how I'd have gotten by with a vegan diet.
I essentially had to go full-on protein/paleo to mitigate symptoms and function.
Candida loves carbs and you may have problems with other fermentables (not exactly FODMAPs, but FODMAPs + other things).
Here's some thoughts to push your MDs on:
If you want to go the pharma route: Xifaxan + Difulcan + Probiotics. Is a good conversation to have. Treating SIBO can cause SIFO.

I literally lived off egg whites and plain chicken for a couple weeks until I could introduce rice and ham and beef (burgers and steak). Strawberries for some odd reason seemed to be just fine.
As far as supplements... the SIBO book has a bunch of ideas... from Berberine, to Neem and Allicin. I've had good results with Neem and Berberine with advice from my ND. I also add probiotics and used IBGard and Atrantil for a while.
I'd avoid IBgard and Atrantil if you have reflux/GERD problems.
I wish I remember where I read it, but there's essentially three methods to fixing a SIBO/SIFO (Candida) overgrowth issue:
Death by Starvation. -> Use diet to feed yourself and not the yeast/bacteria. High protein, very low carb.
Death by Good Soldiers -> Use probiotics to push out the bad guys.
Death by Bombing -> Use antibiotics/antifungals (either pharma or herbal) to nuke the bastards.
I also recommend exercise to keep the MMC working and stabilize mood. If you are having mood issues, it's worth a vitamin check b/c the bad guys can be stealing your nutrients.

Also I would recommend this book: Healthy Gut, Healthy You. In it they provide lots of guidance regarding diet/nutrition, probiotics, and other details regarding dealing with and treating SIBO. In fact, even if you do not have SIBO, the info you will learn from the book will be very helpful given how critical the gut is to human health.

I think the soil based probiotics are different in the sense that they CANNOT be used to make yogurt / kombucha etc. They are primarily present in soil, in nature. I guess our ancestors used to get them from drinking water from wells etc (as opposed to the heavily chlorinated tap water or the highly processed spring water we get)

Boulardi I do not think falls under soil based. bacillus subtilis, bacillus coagulans etc would be soil based

My previous treatments included NAC only and for me that wasn't strong enough. If this is your first treatment, try the NAC alone, and see what you can provoke. If you make to week 2-4 and haven't had a response (headache, joint paint, fever, etc) then you might need to look into a stronger disrupter, like the "Phase II advanced".
The Dr. That formulated it, explains that there are phase I biofilms (weak) that can be treated by NAC... And then there are Phase II biofilms; these are strong biofilms that need a combination of binders and chelaters to break their cellular matrix. If the NAC doesn't give you a "die off" reaction, then I would consider using a stronger biofilm disrupter in a 2nd round of treatment, like the "Phase II Advanced" formula, or Interface by Klair Labs, or theres another one "kirkman" but I'm not familiar with this one.

He says that if you can get away with treating with just NAC, do that. No need to bring out the big guns if not necessary. If that doesn't work, then try something heavier like the Phase II.

I took this one - ( sourced from India ). There are some things that I noted about this one. It’s got trace elements.. magnesium oxide is helpful…the other thing that o noted was that the vitamin C is just ascorbic acid which is what is needed to reduce histamine. The biotin is helpful to heal mitochondria ( again useful in case of histamine). The b12 shoul be ideally hydroxocobalamine. This one does not have it.. the others are also in combo with sulphur which is not ideal for anyone who is having hydrogen sulphide dominant sibo
So the problem here is that in your second piece of advice, you are giving a dosage in mg of OIL, not active ingredient (carvacrol and thymol). Since you don't name a specific product, 600mg of a random oregano oil extract might contain only a little or a dangerously high dosage of carvacrol and thymol. As per this mouse study, while relatively low doses of carvacrol (about 1/3 of what is contained in ADP, if I am remembering right) it has a protective effect on bacterial diversity, but as you increase the dosage (up to 1/2 to 2/3 of ADP carvacrol content) it actually begins to decrease diversity, which pretty much by definition opens up the risk of opportunistic infection and other issues.
ADP is already on the high side, and does come with a risk of disrupting the large intestinal microbiome, which in turn (perhaps very marginally) increases risk of some nasty stuff including the potentially fatal C. diff. An unspecific product dosed at 600mg could be substantially higher than this...
Consider this product: at 600mg dosage you would be getting 360mg of carvacrol daily. At that does you are probably doing physical damage to your GI lining, in addition to absolutely carpet bombing your large intestinal microbiome. Very, very dangerous.

I take either one of these Gut Motility pills, always make sure it has ProDigest in the formula. That's the key ingredient from what I understand (Ginger and Artichoke extract). It's pretty much saved my life, I don't feel like crap everyday anymore and have a much better outlook on life.

I personally do not know if long-term carnivore diet is the answer. Yes you will likely see a significant decrease in symptoms if you go on carnivore diet. You will be missing out on some nutrients though. It is very hard and pretty unrealistic to cut carbs completely out of your diet.
My best advice would be to take xifaxin for 14 days. Then take an antimicrobial like Biotics Research ADP Highly Concentrated Oil of Oregano to really make sure you nuked your system for a week. After that you need to stay on prokinetics for 3-6 months. Motility activator is a good one.
If you do all this it won't return. Trust me.

I was on Rifaximin for many years with largely good success! (although not everyone well responds to it) The latest recommendation is Atrantil (OTC) coupled with a gluten-free diet for SIBO:
The procedure is:
Atrantil only works 80% as good as Rifaximin for me, but it's consistent. Rifaximin would only work like 2 out of 3 times for me, then my body would stop responding to it, then over time I would respond to it again. The nice thing with Atrantil is that it's way cheaper ($40 for 90 pills) & available over the counter (straight from Amazon in the U.S.), whereas Rifaximin without insurance is $1,400 for a 2-week supply, so if you want to test it out, Antrantil is a lot cheaper & easier to get!!
I suspect my histamine intolerance is what has been causing my recurring SIBO. I'm only a month into HIT treatment, so time will tell! My trigger was invasive surgery as a kid; I was sick & tired ALL the time after that! I went off gluten & dairy for about ten years, then got tested for SIBO, tried a lot of different diets & medications, and finally settled on Rifaximin.
My GI works with the top guys in the field (there's only a handful, unfortunately!) & switched me to Antrantil & a GF diet, which has proven to be VERY effective! However, last month I got on HIT treatment & am currently doing gluten (probably shouldn't but it's too good & it doesn't bother me anymore!!). So there are a few options for you to explore, good luck!!

750 isn't that much actually. The brand I used recommends starting at 1400 and then tweaking as needed. I did end up taking 700 at night because it was enough to clear me out in the morning without being so extreme that it would wake me up in the middle of the night urgently. OP- everybody is different. It's an OTC osmotic laxative. It's been used by people for a long time and doesn't cause dependence. Just drink plenty of water because it's going to use it. And be aware the stuff makes you sleepy, which is why I didn't use it for long. But it's not a bad idea to try it out and see how you feel getting cleared out consistently.

These aren't exactly what you asked for, but they worked for me. I had the biggest issue with any kind of tightness around my waist even if it was stretchy. Measurements for my legs were 2 sizes smaller than measurements for my waist, so finding bottoms that fit was impossible. These leggings are not tight at all around the waist because it's a fold over. They're stretchy, but not a constricting kind of stretchy.

It strips the outer shell from the methanogens so they are easier for the antibiotics to kill.
Dr. Pimentel's book talks through some treatment and testing protocols and this book has some good charts and discussion of several treatment options. I was able to get both through my library.
IIRC the elemental diet option was 2 weeks on, immediately retest to see if it has done enough and if not carry on into week three. The books also have some stats to them. And yes, the second one is a cookbook but the first half of the book is all about explaining SIBO and was honestly like having the best parts of this sub all gathered into a book. Really great charts, side-by-sides, and comparisons.
Phytocopeia Ultra Gingerols is a ginger root extract option, being standardized to 26% total gingerols and shogaols.
Perhaps more interesting is a blend of artichoke and ginger extracts. There's Cien por Cien Natural Di-gestión in Europe, said to be standardized to 27,5% total gingerols and shogaols. It's inexpensive too.
I don't know how these fair for gut motility against your German option. Still, this way you know of options out there that you could potentially try.
Could be sibo, but could be chronic constipation. My wife used to be just like this. Taking magnesium oxide each day for a while got her on track again, and then she weened off of it and did other things to promote motility. If chronic constipation gets bad enough for long enough it can eventually promote sibo. If you have no other symptoms you should probably just focus on your motility and get cleared out. Then see how you are doing.

You can do herbals + antibiotics if you want to, but it's pretty aggressive and some people might have more problems with the die off. That's why it's generally not done.
Better to do antibiotics first and then follow up with the herbals if you're still testing positive/symptomatic.
With the antibiotics there are some things you can take to improve the efficacy of the treatment. PHGG is one. The PHGG feeds the bacteria to keep them reproducing and thus more vulnerable to antibiotics. A biofilm disrupter is another, to dissolve the biofilm on the bacteria/archea so it is more vulnerable to antibiotics.
For PHGG a lot of people use SunFiber (I personally can't tolerate this so your mileage may vary).
For a biofilm disrupter, many people use NAC. I can't tolerate this, but the Kirkman formulation is very popular as well. If you already take a digestive enzyme, that might be sufficient also.
No problem, I've gone it without guidance too at times and it sucked. Having an ND give me a solid plan was what finally worked for me, although the collagen and colostrum were my own idea to try.
I take this collagen and buy it from the company, not amazon because lots of reviews said they got weird batches from amazon that didn't work the same as before (maybe they were old?). Type 3 collagen is good for the digestive lining, and type 1 collagen is good for hair/skin/nails. This one has both. I found it on a list looking at contaminates in collagen supplements (heavy metals, pesticides, etc) and this one was highly rated. Also, it's not why I take it, but I do see a noticeable difference in my skin since starting it. It's smoother, softer, and has less fine lines.
I’m not sure what brand the l-glut was, it was the only brand I found at my health food store.
For Colostrum I took this one. It's more expensive than other ones I found, but from what I could gather was the best quality.

I'll put this out here just to help people out. The supplement suggested, GI Motility Complex, is just one. It uses the ProDigest blend, it has apple cider vinegar and rice concentrate as ingredients and it may or may not be available where one lives. What I mean is that it's great to have alternatives, namely ones that could be available in different places or have different ingredients. The ProDigest blend used is available from other brands, such as this Phytocopeia one, standardized to 26% gingerols. Outside of the ProDigest blend, there's this other offer in Europe, standardized to 27,5 gingerols.
Baby wipes? It's definitely time ot get a bidet. i have a simple sprayer that I got online, it's a game changer.

I just use highly concentrated ginger extract: 200mg concentrated to 26% gingerols. AFAIK only one company makes it and it may be difficult to find a product available to you. Usually ginger comes standardized to 5% gingerols, in which case you would want a dose closer to 1,000mg or higher. However, supposedly ginger is much more effective for motility when combined with artichoke extract.

Or just get ginger extract and artichoke extract separately. I think that ginger dose might be a bit low, but then again maybe the ideal amount changes drastically when combined with artichoke. In general Advanced Therapeutics is a pretty decent supplement company. Not sure if this is available in Italy though.
There is also something called triphala, which is an Indian product that combines three fruits that promotes GI health and has been shown to have mild prokinetic effects in some well regarded scientific journals.

My gastroenterologist put me on rifaximin, 2x 550mg a day for 14 days, but it didn't clear it. On this subreddit I was told several times that 3x 550mg is the gold standard.
I then did my own herbal protocol after doing a lot of research (check through my post history to see). You absolutely need to know which kind of SIBO you have before you do this, as the treatment varies depending on the type (mine was hydrogen SIBO).

It's concerning that they don't give an exact MG dosage of carvacrol, just a minimum percentage. It's actually potentially sketchy for your microbiome to over-do it, and if you were to take two of those per day you would definitely be in potentially damaging territory (according to the very limited research we do have).

If you are indeed experiencing AAD, this strain of probiotic S. boulardii has been show to be significantly helpful in peer reviewed research:
S. boulardii is generally well tolerated and typically comes with fewer risks and side-effects than other probiotics, although there are still some risks. Unlike bacterial probiotics it is not likely to make your SIBO worse and cannot colonize your GI tract (when you stop taking it, the yeast will not stick around and reproduce).
I’m going to throw out some of the tips my nutritionist had given me. Hope they help! I have had horrible gas/bloating/pain. Diagnosed with IBS-C and SIBO.
Take 2 of these an hour before bedtime every night. I do this plus 200mg-800mg depending on how I feel. Usually at 400mg these days.
Space your meals out 3-4 hours. If you’re gonna have coffee, have it close to a meal. Water only in those 3-4 hours.
Go for a walk after meals.
Drink turmeric tea before bedtime. Peppermint tea or tablets are also useful.
Not my nutritionist but a friend who recommends this to his patients- try the anti-inflammatory diet.
Take your time eating- should take 20 minutes to eat a meal. Also do some quick breathing exercises before eating.
The Minerva app (gut focused meditation) is proven to help with bloating. It’s said it’s just as effective as the low fodmap diet.
Lay off of HIIT workouts or running. Do low intensity workouts only (low weights or yoga etc). The harder you work out, the more your body focuses on your muscles instead of your gut.
Don’t have too much fiber later in the day.
There is a method to the low fodmap diet. Can’t do it long term. Check out @ibs.nutritionist on IG or other similar folks that provide some free guides.

Hello, I can tell you my experience and how sibo is no longer an issue for me, I’ve been able to eat most everything again, dairy, beans included! Around two years ago, with my naturopath doctor, I did both xifaxan/neomycin after a high hydrogen breathe test. When on the pills, I felt like I was going to die, constantly nauseous. After that I slowly reintroduced food and good bacteria (See below for link to probiotic) it seemed to help a little bit. This is SUPER strange, but I started eating cottage cheese (good culture brand) with blueberries. I don’t know why but that combo seemed to help whatever bacteria my gut was needing. I’m also a true believer that the mind and gut are linked, I’ve been doing therapy for my anxiety and depression, which has been helping sibo issues too. Found out I have adhd. I have been medicating both a bit. Deep breathing and meditation are helpful for me too. I’ve been where you are at, it’s so hard to see the light at the end of the tunnel. Hoping and praying you finding something that works.

I listened to a recent interview he gave on a podcast. he talked about this book being an update of his previous book, from 2006 or whenever.

he also mentioned a new treatment his lab is developing for SIBO that he thinks will prove very effective but he wouldn't give any details. don't know if this is covered in the book, probably not.

I was also in your situation with personality changes from Rifaximin (apathy, depression, increased hunger - see my post history). It went away after the course finished. I was also taking 550mg 2x day, but I've been told by people here and elsewhere the gold standard is 550mg 3x a day.
I don't think my Rifaximin course cleared it, and after my 14-day course things were worse (extreme bloating and constipation). So I started on a herbal protocol, without telling my gastroenterologist, as I was unable to get an appointment with him soon enough. I'm still on the protocol and it has exhausted me from what I believe is die-off. But things are getting better. I'm taking a biofilm disrupter (Interfase Plus), herbal antibiotics (Oregano ADP, berberine), wormwood, and butyric acid.
I would say you should try and speak to your doctor as soon as possible. If they don't help, start researching a herbal protocol you can take yourself, and ask the people on this subreddit for help. Take anti-anxiety meds, establish a diet that's essentially low-FODMAP combined with the Bulletproof Diet, take a 2 week course of antifungals (for candida), followed by a 2 week course of probiotics (https://www.amazon.com/dp/B00JK30A1M and https://www.amazon.com/dp/B000FGXO0W). Neither are listed as DAO supplements, but they are literally made out of the stuff that the DAO "supplements" are (animal kidneys) and may even be higher quality (they're both from grass-fed cows and I dunno about the stuff used in most DAO supps). The major sources of information on the web about histamine intollerance all seem to say that taking kidney (or even eating it if you can, ah, stomach that) supports DAO levels in the gut.
The powder is a little more monitarily viable per dose if you are on a budget but you do get a whiff of the powder. And the powder isn't available on Prime. Both seem to help me though.
Go for the powder if you're really on a budget right now. Do what I suggested above when taking it. If your budget is a little more flexible, you may prefer the pills.
Another way to support DAO levels in the gut is fasting, which is free. Because every time you eat (even so-called low-histamine diets), there is a certain amount of histamine in the meal and some of that DAO (even if it's only a tiny amount) gets used to degrade the histamine in your meal. Not eating for a period of time gives your gut a chance to rest and produce a little more DAO before your next meal.
IMO best supplement for bloating is Atrantil. It got rid of mine ! I tested at 100ppm Methane SIBO in June 2021 to under 10ppm ( considered normal ) in December 2021 and still no bloating or pain every time I eat as of Feb 2022.
It was creating my testing for to reduce methane gas released by cows 🤣🐄. …… whatever works ! It gave me bad stomach pain at first - ai had to stop taking it for a week and re-restart going VERY slowly. Took only 1 pill of Atrantil & 1 pill of Berberine for 5 days or more then added a 2nd one of each, etc. Eventually took 6 pills of each for over 2 months and kept feeling better & better and finally losing excess weight that I never could before too !

If you do not get the antibiotic prescription then take Atrantil & Berberine — that is what I mostly used to heal my Methane SIBO.

From what I gathered, as I was left just as much in the dark as you were, you should be eating some FODMAP while on the abx. Also try using HGG while on the drugs since it has a higher success rate in clinical trials. After youre done, the goal is to populate your gut with healthy bacteria (sauerkraut, yogurt, etc.)

I'm not invalidating your diagnosis. I'm explaining to you that doctors don't actually look for the root cause of things. I wouldn't want to be stuck on mirolax my whole life. Why wouldn't you treat the sibo if you can get off of medications by doing so?
There are three types of sibo, one of which is only just now starting to be tested for. So it is still possible you had sibo before and it just wasn't tested for properly.

I must have real stomach acid issues, because I can eat 20+ of those in a day and have no heartburn.
The relief is totally insane. The other day I ate a giant bowl of yogurt and forgot to take HCl, within 1.5 hours I had all the bad symptoms, crazy brain fog, anxiety, extreme fatigue, feeling depressed. One big takeaway is that feeling fear can stop your brain from escaping these negative patterns.
Since I read it I've been trying to focus on feeling safe, focusing on evidence that nothing is physically wrong with me and my brain is misinterpreting signals as pain when really they're just normal signals that are generating false alarms.

Well, It all depends on your particular case and what works for you. My thought process after the ED was that I would try to keep things digesting fast for another month. Currently, sweet potatoes are doing great, and white rice things like that. However, I have H2S SIBO, and meat/fat and sulfates really affect me badly so it varies from person to person. The main thing I'd say is to keep meals small so the body can digest with its reduced amount of enzymes after ED. Big meals will not digest and will stay in the gut for the bacteria as well as slow down transit time and cause constipation.

Roast veg in beef dripping/coconut oil/duck fat. Potatoes (in particular if parboiled first) are great at absorbing up all the fat and the saturated fat is calorie dense. Plus they come out super crunchy and delicious.
I also 100% agree with all the smoothie chatter, I currently have one. I also use c8 mct oil. Go slow with it to start with but you should be able to get up to 1tbsp with each meal and that's an extra 300 calories right there. It tastes a bit like butter.

I would try a round of Biotic's ADP, Dysbiocide and FC-Cidal or Metagenic's Candibactin AR&BR, either should be pretty effective at combatting dysbiosis. Combining them with a biofilm disruptor like Klaire Lab's Interferase should improve efficacy. Would also maybe use some bentonite clay shortly after to bind the dead yeast and bacteria and carry it out properly. Its important to repopulate the gut with good bacteria afterwards, Visbiome is probably the best option (buy directly from the company).

Not yet but I've finally found the cause. Copper and Zinc deficiency (confirmed with SpectraCell's micro-nutrient test) and methane and/or hydrogen sulfide SIBO. Going on a low histamine diet (SIGHI) and using DAO (NaturDAO) when I eat out have been the most helpful.
