Also you can use something like Microsoft Lens if you have android. Fill out your paper form, take a picture or save it as PDF and you have your electronic copy. Works very well!

There over the counter eye allergy medications that may work. I am sure you could pick them up at any Pharmacy, talk to the Pharmacist even they may offer some good advise too.
