Usually when people are asking this question, the server's page render time is unfortunately more than a lightspeed trip across the world, so I'd suggest you focus your hosting requirements towards CPU and uptime. Use a CDN such as https://www.cloudflare.com/
I'm usually based in New Zealand, and our internet is so bad we really can't tell the difference between an Australian host and a European one for small web pages.
But if you're pushing through 100,000s of hits/hour and/or Tb/hour of data, then you're probably a bit beyond asking r/webmaster
There probably are ways to display ads without the adblockes blocking them, but those ways are usually circumvented pretty quick by the creators of the plugins.

You would have to code your own but it's not hard to find snippets of code lying around the web, adapting them and utilizing them.
PHP:GD is probably what you want. Just take note that if you have shared hosting, image generation uses lots of server CPU time.
