Argentina, France (may take a couple times to connect), Sweden, and Brazil are who I use. Argentina has been the most consistent with a 100% chance of getting ads after connecting, but its connection speed is much slower for me. I am on NordVPN.
They've specifically mentioned that they (Kazam) will not release the list of countries or areas with ads.
I feel the biggest issue is that break chance isn't communicated properly. It's a per-use break chance that adds up the more often the equipment is used, not a per-fight overall break chance.
The more rounds your heroes need to kill a monster, the more often they use their weapons and more often they get attacked - meaning more often their defensive equipment gets used. Let's say a hero has a blue tier weapon (4% break chance) and green tier defensive gear (5% break chance). In earlier areas, you usually kill monster within 1-2 rounds. Later areas could go as high as 4-8 rounds, depending on your gear. For easier math, let's calculate it for 1 round, 2 rounds and for 5 rounds. Link to calculator.
If you finish in 1 round: Weapon 4%, everything else 5% break chance. If you finish in 2 rounds: Weapon 7.84%, everything else 9.75% break chance if the hero got attacked 2 times If you finish in 5 rounds: Weapon 18.46%, everything else 22.6% break chance if the hero got attacked 5 times, which is quite rare though. More likely it's around 2 times (9.75% break chance) to 3 times (13.2% break chance).
The player base at large doesn't know about it, as the game doesn't communicate this properly. To make those later areas cost efficient you need to send in heroes that can clear it quickly because every round they need costs you money. Oh and dodge doesn't help. Even if you dodge every attack, your defensive gear can still break if you got attacked. The only thing that helps is the engineer, as it effectively halves the break chance of your items.
Most just use NordVPN to work around the region restriction. Brazil is one of the countries where it works. I hate ads anyway so never bothered with it, I'm not in that much of a hurry to max out everything (which actually doesn't make much of a difference if trying to out-invest a guy that bought $40k+ worth of gems anyway). It's originally country restricted due to the deal they made with the ad network (fees and payouts are cheaper in some countries than others), something they have little control over. Complain to Google/Adsense, most likely the network serving the ads.
