This is the Apple recommend way to create a real installer.
Then install Clover to the USB stick.
Make sure during the install, you change Install Location and select your USB stick. THIS IS IMPORTANT.
Then click Customize. ALSO IMPORTANT.

Once you install Clover to the USB stick, a new EFI partition will be present and you should see it on your desktop if you have Finder configured that way.
At that point, you configure the config.plist, drop all your kexts into EFI/EFI/Clover/Kexts/other then eject the stick and try booting on your hack on a USB 2.0 port. Boot from BIOS to UEFI:Name of stick and start the install.
This is most likely because you have not updated clover before updating macOS. To get the latest version onto a USB using windows:
Search for CloverISO and download the latest off here: https://sourceforge.net/projects/cloverefiboot/files/Bootable_ISO/ and make sure you download the latest .lzma file and extract it using either WinRAR or 7Zip. Once you have gotten the iso/img file use rufus to burn it to a black USB Drive using ISO Mode (Reccomended)
Put that USB into your PC then select your macOS drive and hit enter, if you have boot commands you need to enter do those first.
Kinda sorta??? I guess is the answer to that.
Patching kexts can still be a pain, and getting Apple's iCloud services to work (specifically iMessage) can cause a fair amount of trouble. But other than that it isn't too bad.
To go into more detail...
There is a new bootloader on the scene called Clover, which provides a much more native installation that talks directly to the UEFI on the motherboard. The previous bootloader king (and still very popular) Chameleon is likely the bootloader you've used in the past. That bootloader emulates a UEFI, and always seems to break after major system updates if not upgraded properly.
Because of Clover's design, it theoretically provides better performance and better compatibility with OS X in general. HOWEVER, Apple started requiring signed kexts in Yosemite as an added security feature. Kinda threw of wrench in the situation.
Luckily the signed kexts security feature can be disabled, but it is still a hassle to ensure that security feature stays disabled. And it still can't be a painful process to find the right kexts and get everything working properly. Audio in particular is usually a giant pain in the rear in my experience. BUT, thanks to Clover, usually everything remains working after a major system update... unless Apple specifically patches something regarding lower-level system drivers.
The other issue people are having is getting iMessage to work. I personally have never had an issue with it. BUT, a lot of people do... especially if they are using Chameleon (but it seems to happen on both bootloaders).
/u/TheRacerMaster has already explained, but I'll throw this out there anyway:
The EFI Firmware update only applies to actual Apple hardware but it won't do any harm to the X220 if you try to install it. It shows up since we're using the MacBook Pro 8,1 system definition in Clover. You should be able to right-click the update to hide it in Software Updates if it continues to show up.
The current Utility and Kext Pack in the X220 Sierra guide has Clover version 2.4k rev 4037 which should prevent triggering this firmware update. Clover has since been updated to rev 4061, so feel free to update if you want the very latest.
This can be done for any OS using clover ( https://sourceforge.net/projects/cloverefiboot/) and the bootloader holding device can be anything your bios will boot from, including cd, thumbdrive, etc.
If you don't have the drivers for FileVault 2, you may be able to recover using the clover ISO and copy the drives from the boot media into your EFI partition, they go into CLOVER/drivers64UEFI.

No. I meant updating Clover (the boot loader), not Clover Configurator (the tool you use to configure the boot loader).
Clover releases are available here. Make sure you've got a Clover backup handy in case it goes pear-shaped (eg. if you have the USB you used to boot into MacOS originally, keep it close).
How are you creating the USB install drives?
Use a cleanly formatted HFS/GUID Journaled USB2 drive in a USB2 port if possible.
Try using Apple's own method "creatinstallmedia"
After the USB installer is created, you still need to install the latest clover onto that USB drive, as well as any special config.plist edits needed for your board, but it can be pretty generic, as you don't need everything to load just to install MacOS.

Also, make sure you've got HFSPlus or VBoxHFS in Clover/drivers64UEFI.
Funny you mentioned that. Their version of Clover is actually NOT the official one. It's a modded one. Here you can get the official one: https://sourceforge.net/projects/cloverefiboot/. That's the official website.
And if you install this version (which I definitely recommend), pay attention to the installation flow. You will see this version has a Customise button, which allows you to install exactly what you need.
And please, please, stop downloading stuff made by other people. Software evolves. And, maybe in this case, the hardware only comes in one variant, for Desktops, those EFI folders and all those pre-built solutions are not guarantee they will work on your particular rig.
That's why forums like insanelymac.com (which I would totally recommend, there are some awesome people around there) don't have it, and don't recommend it. Cause what's good for my rig might not be good for yours.
Also, as I said, software evolves. Software changes with almost every version. After all, that's the point of the update. If nothing's different, why bother? The stuff you needed to boot Yosemite and El Capitan, for example, is NOT the same as the one you need to boot Mojave and Catalina.

Unfortunately, a ROM update...assuming there is one available for your card. And, in order to make sure the ROM is compatible with your specific card, you might wanna request it on the manufacturer's forum or something. Ooor...if there's a management tool for your card, from the manufacturer itself, which allows you to upgrade your VBIOS, that could work, too.
But don't attempt to just flash a random VBIOS from the internet. Even if it's for the same card.
Are you able to try Windows? Just to see if the same thing occurs there? Also, I don't remember if I told you this, but just to make sure this is not some sort of weird MacOS issue, could you try to install MacOS on a clean partition? Preferably the "vanilla" way. Meaning using "createinstallmedia" method, Clover downloaded from their official website, and no "beasts"?
What's your current Clover revision? You can boot in verbose mode and see exactly where does it hang. Clover update might be the solution, but it depends on what's the problem first. Also, your FakeSMC kext might need to be updated, as well.

But for that, you need access to the EFI partition, which means you need a booted OS. So...you can make a Clover USB drive on another machine, and use that to boot your current one. Aaand...if you have Windows installed, you can also install Clover on a USB drive, from Windows, using cvad's boot disk utility.

I was unable to get High Sierra to boot with VNC video and gave up. I generated the USB installer using the spaceinvaderone guide and my roommate's Mac.
Here's the notes I gave my roommate on first attempt:
- Download High Sierra into the store app as usual (done?)
- use HD utility to erase a USB key on the Mac, HFS Extended (Journaled) GUID Partition Map
- use macdaddyio install disk creator, target same USB key
- download Clover https://sourceforge.net/projects/cloverefiboot/ - Run Clover to make Steve Jobs roll in his grave: - Change install location: our USB flashdrive - Customize - Install for UEFI Booting - Install Clover in the ESP - Drivers64UEFI - OSX APTIO FIX2 - Partition DXE 64 - apfs - HFS+ - Install to USB...
- In addition to the USB installer drive mounted on your desktop, a new volume called EFI is mounted to your desktop, open and navigate to EFI > Clover > kexts. Open the latest release in the list, I'm guessing it's 10.13, navigate into there. Unzip the folder FakeSMC.kext from the zip file attached and place the unzipped folder in that path.
- Finally, copy the downloaded Clover files and the HighSierra_Support_Files (unzipped) to the root of the USB installer drive (not the EFI image)
- Unmount the EFI image, then the USB key. Should be good to go.
Optional, probably not needed: - A new volume called EFI is mounted to your desktop, open and navigate to Drivers64UEFI. Place the apfs.efi and HFSPlus.efi (attached) in same. I suspect that the current release of clover has done this already.
I then followed /u/spaceinvaderone's video install guide, fiddled with the BIOS VGA res, never got it to boot with video. Any tips? Roommate leaves with his Mac in 2 weeks but would love to get this working.
I can try to help you with that set up. They are mostly all the same. The usual kexts and config.plist settings. Then minor differences depending on graphics card and audio.
The first thing I would recommend would be getting rid of that Unibeast installer and creating a proper OSX installer.

Um... So - what most people do is use an existing Apple computer to download the "Install OS X El Capitan.app" and then build an installer from there. If you don't have access to an Apple computer - the next step is to get OSX installed in a VM, and use that to build your USB. I have seen the website Olarila.com mentioned from time to time - otherwise you could download a distro (I know... heresy - they aren't supported here) and use that in your VM to get your vanilla installer created.

So - nv_disable=1
is a boot flag that tells OSX to load some VESA video drivers just for compatibility's sake (the Maxwell NVIDIA GPUs [9XX series] are not compatible OOB with OSX). One you install NVIDIA's Web Drivers, though - you'll need to tell the OS to load those instead of the VESA drivers; that way you get full resolution, hardware acceleration, and QE/CI. That's where nvda_drv=1
steps in. You want to remove nv_disable=1
when you add nvda_drv=1
- otherwise it'll still load the VESA drivers.

Try just using the actual Clover installer. Test under 10.9 before upgrading to 10.11. Format your USB drive before installing it. You'll want to check:
Install Clover in the ESP
Bootloader -> Install boot0ss in MBR
CloverEFI -> CloverEFI 64-bits (try either SATA or BiosBlockIO, but not both)
All Drivers64
Then once you get it installed, copy over essential kexts into your USB's EFI partition under EFI/CLOVER/kexts/10.9 (move to 10.11 after). If applicable, copy your DSDT and SSDT over to EFI/CLOVER/ACPI/patched. You may also need to edit your config.plist depending on what boot arguments you use in Chimera/Chameleon currently. Once you get your Clover working properly, install it to your boot drive, copy your 10.9 kexts over to 10.11, and you should be set to upgrade to 10.11. Make sure you do a full backup of your 10.9 install with SuperDuper or Carbon Copy Cloner just in case things go south during the upgrade (or if you just don't like El Cap).

I use Clover to boot an R720 off an NVMe SSD. I used a cheap flash drive plugged into an internal USB port and copied over the contents of the ISO. I had to enable UEFI drivers for NVMe so you may have to tweak the configuration a little
What's the full command that you're using?
Are you trying to just run qemu-system-x86_64 or virt-install? Or are you using virsh or virt-manager?
I personally haven't used edk2-x86_64-code.fd but have had great luck with the Clover EFI boot loader.
In my experiences, clover has been the way to go, but there is no official support (the newer versions can, but it isn't tested that much) of big sur with clover. Opencore is kind of a new thing that people are trying out but requires much more work. Instead of Unibeast (if you don't have compatible parts) I recommend going here and getting a copy of the clover installer if you already have a mac. Then install clover on the USB stick with the kexts and drivers, and name the non-EFI partition to Install macOS XXX (Catalina if you want to use Catalina, ETC.) Then, you move a copy of the macOS installer to the non-EFI partition, configure the bios, and boot up. Sorry if that was long, but I have had some trouble with Hackintosh before and wanted to be as clear as possible.

Bootcamp is Apple's bootmanager that can run windows OS.
However, there are also 2 open-source bootmanagers available that cann run Mac OS, Windows and LINUX on any Mac hardware.
What version of Clover are you using? This could be the text generated by the old apfs.efi driver.
Check the EFI/CLOVER/drivers folder (or EFI/CLOVER/drivers64UEFI if you have an older version of Clover) on your EFI partition. If you find apfs.efi, replace it with ApfsDriverLoader.efi available here. You should probably also update Clover.

I think the latest official release 4961 might have some issues booting Catalina. They have been fixed in the upcoming commits, but there is no official build yet. Only Beta builds. So until then, try 4945 with Catalina. When new release comes out (version number should be newer than 4964, which was the commit that reverted the issues introduced for Catalina) you should be able to upgrade with no issues. Until then, use 4945. Please, let me know if it works.

Let's try 4741 for example. First step would be to load Clover. It might not work with Mojave. Right now you're not even getting to Clover GUI, right?

Right now, I'm really confused. But for as far as I know, this issue is related to the Clover version used.
If you don't have an iGPU, Inject Intel should probably be Off in config-Graphics. Also, could you please, try to disabled the rename for GFX0 to iGPU? I don't think you need that. You can enable it later on if you want.
Also I don't know where did you get that config, but your SMBIOS is awfully empty. Could you try to select it again from the drop-down list in SMBIOS so it populates all the fields again?
And no need for a SSDT-NVIDIA-Disable.aml...since you're not using an Nvidia card. So there's nothing to disable.
Also, you have a -disablegfxfirmware... But that's for an iGPU... Which i don't think the E7400 has. So...I don't think you need that boot flag either.
However, you might need -no_compat_check boot argument instead for the CPU.
Also, if you've populated your kexts/Other folder with kexts, no need to put them in the 10.14 folder, too. Actually, you better not do that. It's confusing and harder to maintain. :)) Clover/kexts/Other is the only folder you need (unless you need different kexts for different versions of MacOS, which is not the case here).
Also, your Clover looks a bit old...? You've got some really old drivers in there. Haven't seen them in a while. Could you please, update it from here?
That looks like iMac14,2. And usually no. But I've had friends with issues with different SMBIOSes (such as iMacPro1,1 for example) which then worked fine when switching to iMac18,x for example. So...usually, no. It shouldn't. But my experience says it can, sometimes.
Hmm...have you used some kind of TonyMac tools? Your Clover seems a bit empty for a hack. :)
And yeah, you could copy-paste your EFI from Mojave and see if it works.
BUT, before doing that:
I've already got newer versions of those kexts so I can share them. But if you say you don't use them...which sounds really, really strange, then...maybe you don't need them. I guess we'll see.

Try to download and reinstall it onto your SSD/HDD.. Make sure you're installing it in UEFI mode (first two checkboxes in the Clover installer need to be checked), also select drivers64 UEFI. All the ones already checked are mandatory. You will also probably need ApfsDriverLoader and probably others too.
Also, is your HDD correctly formatted as GUID (with HFS+/APFS partitions)?

Jokes apart. My advice is to follow the vanilla guide that is much better than premade images. I have a Dell Inspiron 5370 with Mac OS Mojave 10.14.5 (latest release downloaded from App Store) that is installed using the vanilla rules.
Basically for a Vanilla install you'll need a Mac OS Environment where to download the latest Mac OS Mojave version (such as a VM, view techsviewer guide) using http://dosdude1.com/mojave/ macOS Mojave Patcher - Dosdude1.com, TINU https://github.com/ITzTravelInTime/TINU and Clover https://sourceforge.net/projects/cloverefiboot/.

Yeah, everything looks good, in my opinion. And yes, pretty much everything should just work OOB. Good choice on the graphics card, too. But, as dracoflar said, we would recommend you to use the vanilla guide posted on the side bar, instead of Tony tools.
With that graphics card, iMac18,3 would probably be the best choice for the SMBIOS. Use Clover Configurator to change it in case it's something else (which, since you used Tony tools, it probably is). Also, for future Clover updates, this is the Clover Bootloader's official page.

Once you get your OS installed, you might also need to do some cleaning to your efi kext folder. Since I've got the feeling that it might be cluttered with a lot of stuff you don't need and never use. But I guess we'll see about that when we get there.

So looking through everything you provided, there's some serious issues with how the Hackintosh was setup. Specific issues I notice:
For now I'll avoid this but I'll get this fixed for you in a bit, is there any other issues with this Hackintosh? With these settings, there's likely issues with AppleID and other services not working, which could interfere with how the thunderbolt Controller operates
And with the thunderbolt controller, there's likely an issue in BIOS settings.Try the settings recommended in this post, and if it doesn't work try updating the BIOS.

That’s absolutely bizarre! Try using this clover: https://sourceforge.net/projects/cloverefiboot/ It worked for me. Also you didn’t say where you got your copy of Mac OS, but if it wasn’t from actual Mac hw, that might be it as well. I had a bugged copy from a VM running OSX and it kept crashing and such. It’s possible something like that is happening.
Ehhhh I wouldn’t use multibeast or unibeast. They could cause problems. Try to make the installer using install disk creator.

I linked to that other one because it has the BIOS settings and a complete EFI folder with all the kexts you'll need as well as a working config.plst file that was written for your Mobo.
You need both the bootloader and configurator - the configurator helps adjust the bootloader post-install. When you use the bootloader's install app, it should make an EFI partition for you. Note that the EFI partition won't be visible on your desktop, but if you fire up terminal and enter 'diskutil list', you should see the EFI partition listed.

The version you're on is cropped in the photo, but it appears to be r40xx which is pretty old. The current version should correct this problem.

Use newest clover package, use AptioMemoryFix.efi in UEFI mode and make sure that you have no slide= boot argument. Also, you probably need to enable some power management patches.

EDIT: Are you injecting FakeSMC.kext? You probably did not configure correctly, there are all sorts of guides to get those components working...
What version of Clover are you using?
There was an issue with recent versions of Clover and macOS 10.13.4 that caused exactly the problem you are describing. This has been corrected in the current release of Clover (r4428) available here.
You can get Clover EFI and install macOS on Windows hardware.
For two years my main systems were a home-built Hack Pro tower and a HackBook Pro. They worked very reliably and could handle updates without any problem(as long as I made sure Clover and the extensions it used were updated before updating the OS).

At this point, you would add your kexts and adjust your config.plist.
But here is an EFI that will work with your hardware.
Download your EFI.
Copy the EFI folder to the EFI partition of your newly created Clover/High Sierra install stick. REPLACING what is there.
Eject USB EFI then eject all. Place in your hack, in a USB 2.0 port if possible. Boot from BIOS to UEFI:name of stick.

Handoff Continuity will not work without BT working. Assuming the card works, I see no reason why you can't get BT working with vanilla. It is possible that Multibeast did something to the stock kext within the system that manages Bluetooth.
I think vanilla is the best way to go by far. Do you have a USB stick 8GB or more? Download the latest Sierra from the App store and the latest Clover to start.
Ok, I did that, and installed the latest version of clover, but I still have this same issue. What exactly is this doing? Just updating the newest version? I'm confused here. Appreciate the help.

Well - it depends on how you set things up. Uni/Multibeast use a custom Clover setup, so you'd likely have to figure out what setting work best for your machine before installing the official Clover package over the top (if you used the TMac tools). The Clover install package can be found here.
4061 broke booting on my Haswell system. Reverting to 4049 got it working again. Could be worth a try, although doing that alone won't fix a configuration problem in Clover itself.
Getting things fixed required setting up a USB stick with 4049, booting the box with that, then running the 4049 installer on my system drive.

Only caveat was how to mount the EFI partition, which I already knew how to do before this whole adventure. I think Clover configurator (separate download) can do that for you, but I knew how to do it manually.
You need to completely format your stick, then create the vanilla. Format GUID Mac OSX Journaled.

You'll need to create an OS X installer. There are tools to automate this, but it's best to learn it for yourself because every hackintosh is a little different.
The basic idea is to use createinstallmedia like so:

The default config.plist works out of the box usualy. If not set your smbios to 14,2 inside clover configurator.

I actually wrote some tools that do EFI mounting (otherwise you can use the Terminal). You can find some info on my github (Mac Multi-Tool [and the GUI version] can mount EFI partitions - I also included a script called MountEFI).

When I do a fresh setup - I use the Clover installer, download the individual kexts I need, and edit the config.plist with TextEdit (just make sure to turn off all the "smart" features in preferences).

I always install the kexts into /EFI/CLOVER/kexts/Other/ as that folder is more reliable than the 10.xx folders.

Run this in Terminal:
defaults write com.apple.DiskUtility DUDebugMenuEnabled 1
Open Disk Utility. Go to the debug tab, check for "Show all partitions". If there's an EFI partition, good, skip the rest of this paragraph. If there's not, format your USB with 2 partitions - named EFI and El Capitan. Make the EFI partition 350MB, and use the rest of the USB drive for the OS.
Download Clover from here, install on the EFI partition. If the EFI partition doesn't show up when choosing where you want to install Clover, just choose the El Capitan partition. From my experience, it just makes a shortcut to the EFI partition on the main partition then installs to the partition it's supposed to be on.
