A2DP Volume is the best solution I was able to find when worrying about exactly this with my BT earbuds recently. It works pretty well even if the UI design is, like, ancient by now.
A2DP Volume is a fantastic app which handles custom bluetooth volume issues. Its free (and add-free), but the interface is old-n-clunky, which is fine as you only use it to setup your devices.
You can set each bluetooth device to have its own initial volume (for both music and call streams), you can set it to read out text messages (or other notifications), you can have it automatically turn off WiFi, and it'll even remember your GPS location on disconnect (so you can find your car again). You can also launch any app - or shortcut - automatically on connect. And, of course, on disconnect everything is undone and your phone settings return to where it was.
In addition to setting bluetooth volume, I use it to:
Turn off WiFi when my phone connects to my truck. My phone is still - barely - connected to home WiFi in my garage, which means that its got a crap connection when I'm trying to find my destination and use Google Nav; I'd rather be on data.
automatically read out text messages and... *sigh* facebook messenger... when I'm using my bluetooth earbuds, but NOT when I'm in the truck (and someone else may be in there as well to overhear a private message)
remember where I parked my truck to a 30' accuracy (yup, you can even set the GPS accuracy)
when I connect my bluetooth selfie stick, it automatically opens my camera app in front-facing mode so that its ready to take pictures; and when I disconnect the selfie stick, the app is automatically killed
And, back to bluetooth volume: I use it to jack up the volume to 100% for the truck and the BlueBuds X2 ('cause both devices handle the volume levels themselves), but only to 60% for my bluetooth speakers and snowboard helmet ('cause their volume levels are sync'd to the phone).

Not true. Cataclysm work just fine. Including WiFi calling.
Its a problem that was introduced with 6.01.
it hasn't really affected me too much - my volume control still works, it just starts out way too loud when I connect with some Bluetooth devices (Bluebuds X2, Jumbl 4.0); other devices are unaffected (JBL Charge, Bose SLIII, the KS-BTA100 in my truck).
Fortunately, I can config connect volume per-device using A2DP Volume - an app that I love and I've used for years; the UI is ugly, but you only use it to config. But, I doubt that app will work for your devices if you can't change bluetooth volume on the phone to begin with.

HFP profile will not behave properly with your galaxy 5 phone (known android lollipop issue ) as this btfm 4 has a 4.2 version of Bluetooth.

Same problem here. I use A2DP Volume as a solution. It's a bit wonky sometimes, but works 90% of the time.
