That's why if Google publishes an update for that app that includes ads, all people with it installed (including Shield users) will get that update.
Nvidia could make their own launcher and ship it with their Shields if they wanted to.
You can also just sideload a third party launcher made for Android TV if you're a current Nvidia Shield owner. Should work fine for Fire Sticks too.

So far, our Sony Android smart TV has no ads in the OS, and even supports sideloading, meaning we can install apps like SmartYouTubeTV and get no ads there either. You could also just plug an external interface into a "dumb" TV. It's hard to find a dumb "TV" in over 1080p, but you can look for large monitors, commercial displays, and "large format displays", which can act as the same but with a different name.
EDIT: On second look, I think ads on the Android smart TV were added to the launcher (home screen) in an update, but I did not notice, as they are not that intrusive… but more importantly I don't even use the TV that much. I will have to look to see if I can sideload an alternative launcher, since so far, it seems Android TV can do everything else phone Android can do.
Edit 2: It was definitely an update, but not to the OS. I can't find a recent open source launcher, and upon searching for the original from Google, apparently Google is pushing this junk themselves. And, that is the version the TV is currently using. How horrible. I wonder if they are going to start pushing something similar to the Pixel launcher? We really need an open source Android TV launcher, because it could be used, if only it existed.
Edit 3: Found a Reddit thread about it in /r/Android. This antifeature was apparently added only about 1 month ago. It's no wonder seemingly no one has bothered to make a libre alternative yet!

It looks like Android TV Launcher is available on the Play Store. Not sure how well it will work as I'm on mobile, but it might be worth trying.

source, search, heart and pencil: the "heart" can be accessed from the menu. Press the 3 vertical dots then press "Bookmark" or "Remove bookmark". It fixed the problem for someone else with an android tv. I'll add source, search and pencil in that menu as well.
Calendar and MAL: I'll investigate. The list of series, latest, ... are working so I don't understand why these 2 don't.

Yes, the Leanback Launcher (play.google.com/store/apps/details?id=com.google.android.<em>leanbacklauncher</em>) is what the launcher is called by our community, but asking what lean back is like OP's post is what I was answering.
Seconded to /u/Dsk001 . Android TV is effectively just a custom launcher, the leanback launcher, beyond that it's just standard android under the hood, install the leanback apk and, you've got an android TV.
It would probably be easiest to stick with ARM-based devices that already run android (as opposed to a NUC/x86 device) for compatibility reasons. Easiest solution would be to get a nexus player but, if that's out as well, there are a lot of boxes that run base android out there that you could make work. The only difficult part is that you're going to probably want something that can run Android 6/Marshmallow, I don't think the leanback launcher is compatible with anything earlier.
