FIXED IT! The problem was I had a second magisk app. I hided it and disabled it using this to hide root from banking apps as I already mentioned. That's why I couldn't see it. Lucky I had LS Posed and xprivacy module installed. I used them to uninstall the hidden app from there and everything worked!

Just be careful what services you disable, especially with Google Play Services.
On my Verizon G3, my stock app drawer has a menu with a "Hide/Show apps" option that you can use to hide things like Setup: http://imgur.com/Kzg6Ibe. (If you don't have the 3-box menu button, try tap-holding the app switcher soft key) This works in both KitKat and Lollipop. Another option if you're rooted is to use something like Disable Application (https://play.google.com/store/apps/details?id=com.iamaner.oneclickfreeze) to freeze the LGSetupWizard app (com.android.LGSetupWizard)

Go to your renamed Magisk and disable it after you finished with app undisable Magisk!

I did it with Magisk module 'Debloater' first, but I later found out that SafetyNet is tripped with this one, whereas the SafetyNet check passed with this app. Haven't gotten the notification since!

I chose this over updating because I don't want to lose root, and flashing an update is a bit daunting for me as I am relatively new to rooting and flashing etc. Guess I'll update when an update seems good enough to be worth losing root access.

I personally can't disable the cyanogenmod camera (in this case, snapdragon camera) for somehow. I end up using this app to freeze and disable.

PS. I think if you uninstall it you may have it again in the next nightly you flash (correct me if I'm wrong) if that the case then just disable it.
