Well i guess you can use portal too but this app has very clean layout. You don't have to remember which exercise to do next, you can set up automatic rest time between and after exercises. It's very convenient. If you got a min check this out.

I've been using this for almost a year and love it. Its got a pretty solid selection that you can create your routine, has the timer like 5x5, can adjust weight and reps on the fly. It's done a great job for me; only downside is it doesn't have the nice charts like 5x5, but if you really care you can use sheets like you said.

If you mean you don't know what to do as in exercises, just follow a workout plan. There's a lot. If you mean you don't know how to do the exercises, use the machines.

Also there's a really well made app called "Workout: Gym Personal Trainer" by fitness 22 that shows you short gifs of tons of workouts for each muscle group. It's great for people just starting out.

And just under the my trainer, there's a pay option. I don't think this is on iOS.

This app let's you do that. You can also make your own program and track progress. IMO it's pretty good
