He probably made this with the Hexacon icon pack, and KLWP.
Probably using some kustom komponents (Yes this is what they are called in KLWP) as well for stuff like that battery meter.
Hey guys! Long time lurker, but this is the first time I've built my own theme from klwp! I couldn't figure out how to incorporate a music player, but I think I am very happy with it as it is. Here are the resources:

Edit: Oh I forgot to mention this is Nova launcher with the app drawer set to scroll from the bottom with the Google now integration so you can get Google now on the left side
That icon pack is Hexacon, by the way. I use Tendere from the same developer.
Inspired by /u/jordandrako's take on Overwatch yesterday, I decided to make my own version.
The header shows the current day, date (tap action: calendar), time (tap action: clock), and battery (tap action: settings->battery). Everything is anchored top-left so it should be resizeable. Its designed to be full-width.
The main navigation can be any width, and there are tap-target overlays over each word which are also full width for easier usability. They are currently configured for the default apps.
The ability bar (skewed icons) is set to show current temp and conditions plus a link to the alarm. This is fully customizable. Just make sure your icons are layered below the skewed box. Set your tap target on the box itself.

Launcher is Nova Launcher Prime. I use a 6x5 grid.
Background is included with Hexcon icons.
Download: Google Drive. There's a blank iconbg for the ability bar if you want to create your own icons.
