This is the template for the clock widget.

Unfortunately I can't save the template for the other widgets as the author won't allow it but here's the link for the Trident Pack (it's on sale now apparently).

From the Trident pack I used

lose angeles. I removed the time box, and shifted everything up

Royal Vintage. I removed the date and day, and changed the time to show battery and locality.

Top clock/battery bar widget is a custom zooper widget made by rusrslythatdumb and customized a little bit more ;).
Middle widget is a zooper widget from the Trident pack.
Icons are from the Iride UI pack.
