Affinity Designer is another option as well. It has some more features/capabilities than Inkscape, but is still much cheaper than Illustrator ($55, no subscription).
I suggest starting with Inkscape, and as you get more experienced you may decide you want one of the alternatives.
It's perfectly possible. The map shows services rather than actual tracks. There is a service from Rome to Rimini that does not stop in Ancona, but it actually changes direction in Falconara because the line over there would naturally go towards Ancona instead :)

I would also start out with the openstreetmap or e.g. this map, and then use 0°, 30°, 60°, 90° degree grid to force / draw first the basic outline of the island, then the subways, and then the bus lines. Then I would spend a hell of a lot of time on placement and and aesthetics.

I was digging around on an old hard drive, and I found a block of my oldest work, back from when I was starting to teach myself how to use Inkscape in 2008. As one of my first attempts, I made this map of the Sacramento (CA) light rail system. (I've since switched to Adobe Illustrator, but Inkscape can produce excellent results as well - and more importantly, it's free.)
It's nothing special - two lines, no real attempt to show the geography, but it's where I started from.
I use figma, however I don't really share my maps so you can't judge it here. It's requires free account (you can log in thru Google), and works as as app or in browser (although I had better experience with app)
I heard its similar to illustrator, but idk of thats true. Its mainly for making ui for apps, but I find it as a less confusing inkscape alternative. I highly recommend it.
