I’ve struggled with acne since I was in 4th grade. I’m 22 now and have tried a plethora of things like changing my diet, hormonal birth control pills, topic prescription creams/ointments, and drugstore acne products.
The top photo is 2 years ago (at my worst), the middle photo is 1 month ago, and the bottom photo is today (9/14/2020).
Here’s what helped me:
Microneedling every 3 days with a vitamin C serum. This is where I saw the most results, but it’s not for everyone. I highly recommend a 0.25mm derma roller but would thoroughly research it and consult a dermatologist. If done incorrectly, it can cause a lot of damage.
Sunscreen (I like EltaMD) EVERYWHERE. ALL THE TIME.
The Paula’s Choice skincare line is also wonderful and I use the 2% BHA liquid exfoliant after cleanser + toner.

Could it perhaps be hidradenitis suppurativa rather than cystic acne? Having it on your butt and genitals makes me think there’s something else going on rather than the acne alone,

I used to have really bad keratosis pilaris on my arms, some spots on my back around my ribs. I exfoliate with this mitt twice a week. My skin is completely cleared and soft as a baby's bottom. Don't use with soap. Let your skin soften in the water for a bit, then dampen the mitt and scrub in circles. It will be rough and a bit uncomfortable if you're not used to using such a coarse texture, but you'll see your dead skin balling up and coming off. If you've never done a deep exfoliation of your body, you'll be surprised how many layers of skin and gunk just stick to us and build up. Hope this helps. I've been through trial and error with so many other mitts, sponges, towels meant to help with keratosis pilaris but fall short every time or wears down before it's paid for itself. Are you taking synthetic B12? Reason I ask is there are two forms of B12 - the "synthetic" version is cyanocobalamin. The preferred formulation is methylcobalamin. The cyanocobalamin version is what's sold on most drug store shelves and is made with cyanide and cobalt and our bodies must do the work to convert it to the preferred formulation. The excess cyanide and cobalt must detox from our bodies, which makes your skin freak out. If you feel you must take B12, I would suggest getting one with a methylcobalamin formulation. I'm 34F and take the Smarty Pants for women gummy vitamin which has B- Methyl formulation. They make a mens version too:

But, with that being said, regardless of which formulation you take, too much B12 can trigger acne, so if you're really borderline, you may want to not take it every day as your body could be feeling a B12 overload :)
I say first try Nizoral shampoo. Put a dab on a cotton ball and put a thin layer where it's needed and leave it on for like 15 minutes a day and it should be gone in a week or two. It does leave a sting if you leave it on for too long just so you know. I did it for about 2 weeks and the bumps haven't came back since.

If that doesn't work, try something with a chemical exfoliant in it. As long as your skins not super dry or sensitive. Probably a face wash, like Cerave Salicylic Acid Face Wash. Or you can opt for something like an acne-fighting cream thingy or a serum. It's very mild so you shouldn't have to.

Yes I have as well. It causes a little irritation for the first few days but I find the benefits are worth it.
You can try body washes containing salicylic acid too and see if that would help. Many people surprisingly do not wash their backs. If you don't own a back scrubber or a wash cloth like this one, you might want to add them to your toiletries.

Try Gold bond medicated powder, it does more than it should, probably cause the zinc and thymol. You could probably get one of those powder applicators and a brush so it is a thin layer that wouldn’t get on your clothes too much.

I really struggle with cuticle picking. I found this Neutrogena product with salicylic acid and kaolin white clay that can be used as a cleanser or a mask, and it has pretty much cleared up my hormonal acne, gotten rid of my closed comedones, and it’s even starting to fade my PIE/PIH.

It’s called “Hada Labo Gokujun Hyaluronic Lotion”. Japanese people call toners as lotions so the name is confusing. But I live in Japan and it’s only about $5 here whereas in US Amazon it’s nearly triple the price so I don’t really recommend buying it in the US because it seems that CeraVe has their own version!
Dab 5-7sh drops into your palm, rub your hands together and apply and pat onto your face. Pat into it’s sticky ish and wait until it absorbs into your skin to apply your face lotion.
As for working out, I would use a cleanser cause if sweat mixed with SPF makes me feel bleh. Your remaining SPF lotion may still need embedded into the skin and I wouldn’t want to apply toner after that.
If you didn’t apply your lotion with SPF before working out, rinsing thoroughly is OK I think.
CeraVe’s cleanser doesn’t seem to be harsh from what I remember when I lived in the states, but, I think what my esthetician and dermatologist would do is use a cleansing oil instead. You’d keep your skin moisture but also remove everything else.
Best answer would probably be:
SPF (then work out) -> cleansing oil -> toner -> moisturizer-> SPF
Didn’t apply SPF prior -> water rinse -> toner -> moisturizer -> SPF
Also, I’d avoid taking too hot showers and letting the water hit your face for too long. It’d dehydrate your skin faster and acne marks’ redness lingers much longer.

Technically yes. Some of it may be hard to apply but it's worth a shot IMO. I'd suggest maybe washing your back with Kojic Acid soap. It works similar to Hydroquinone which would help any spots or pigmentation. If you struggle to apply acids (since they are liquid) you can try any lotion that is advertised for 'bumpy skin' because they usually contain a mix of AHA's and BHA's.
I've also used a tea tree ointment on my husband's back and it helped. Just try to get one without essential oils like lavender.

These are what I use for anything that's come to a head. They're non-medicated and drain/flatten bumps overnight. It's Hermione Granger-level sorcery when I get breakouts like your photo. I have them on subscribe & save.
I do have salicylic acid blemish dots from Skyn Iceland, but I typically use those when it's just starting to form before it comes to a head. Success is hit or miss; sometimesit brings it to a head so I can gently drain the infection, cleanse and proceed with other treatments.
If a patch doesn't help, I use the Mario Badescu Drying Lotion overnight, which successfully dries out the pimple, then I rehab the skin with extra moisturizer in those areas. When you buy this, be sure to get the plastic bottle, not the glass.
On top of this, I use a 2% salicylic acid toner after cleansing 2x/day. Seems to mostly keep things under control, but then again, I'm not currently dealing with the stress of planning a wedding.

I’ve used “natural” skincare many times and haven’t seen anything work well, such as topical treatments. I’m talking tea tree oil, pink clay, Aztec clay etc. I just take B5 supplements now, which another name for it is Pantothenic Acid. I’ve been on many oral acne pills like accutane (two times in 10 years, so obviously not a cure all) and mino/doxycycline which are antibiotics that you should not be on for a long period of time. I’m so sorry!! I do however use a body wash from cerave called “cerave SA Bodywash for rough and bumpy skin” I read that people use it for bacne, so I’ve been trying it out. With pcos as I have my eostrogen is too low and the testosterone is too hight. I take inositol supplements to help regulate the hormones. It's obviously not a magic wand but it's definitely helped. I also had a mirena coil (Hormone regulating IUD) what was AMAZING but again I it depends on whether you have ovaries/uterus as to whether it helps or not. If you don't have ovaries/uterus I can't really reccomend something for you but im sure there are similar supplements for hormone imbalance for testes related issues but I don't have first hand experience with those so I can't really help in that circumstance. (Talking sex characteristics not gender) I hope you find what works for you. Reddit has been a big help in my acne journey. any authentic african black soap is worth a shot, you could also look into some pure wholesale(better price) rose water. i put that in a spray bottle and spray my skin after cleansing or whenever i need a refresh. I am completely going through this same struggle with my own acne! Obviously going cold turkey on any habit is challenging, but a couple ideas to minimize your picking would be: -set a timer for yourself on your phone before you start, stop at the timer (maybe a minute?) -designate one time a day when you can inspect your face a little closer, ideally before bed so your skin can relax before seeing anyone (the redness/obviousness that I was picking makes me self conscious, too! -keep your nails trimmed -every time you find yourself with the desire to pick, do something else instead. For example, drink a glass of water. Refocus your mind, stay hydrated (which will help with your skin) and reset that connection in your brain with something else. -spiky sensory finger rings as well as zip tie fidget coils have been really helpful in keeping my hands busy throughout the day

As far as my personal experience adapalene is not so helpful for fungal acne. It has helped my cystic acne tho. I suggest using products with zinc in it. I used The Ordinary Niacinamide 10% + Zinc 1% Oil Control Serum and a microderm vacuum to make the product go in deeper into my skin.

Ooof 3rd time Accutane is a rough! :/ DIM is a supplement made from cruciferous vegetables. You can get it in different dosages from 100mg to 300mg I think. Endocrine problems can cause so many hormonal issues! I've been taking it for 3 months and I was shocked at how well it worked. It also completely cured my quite severe pre menstrual breast pain.
The only thing I would advise is that because it flushes estrogen out so effectively a side effect is possible migraines when you first start. I noticed the first 2 weeks I was getting a lot of headaches and then they subsided completely. If that is something you experience you could start with 100mg until your body gets used to it and then increase the dose. Honestly for $30 bucks it's really worth a try :) Just be mindful that supplements aren't regulated so it is important to buy from a reputable brand. Best of luck! :)

i tried that specific neutrogena benzoyl peroxide product at 5% and it gave me an allergic reaction, so if you try it i'd recommend a patch test first- i underestimated how powerful it can be. i have since ordered this cera ve cleanser that is 4% BP and have been using it carefully with no problems. good luck!

I got off the pill for a year and felt so much better in every way, but got rebound acne a year in. Only thing that cured it was a low glycemic diet, benzoyl peroxide+clindamycin 5%/1% gel, and some supplements she recommended (magnesium, zinc, DIM, spearmint tea).
Unfortunately not :( there are a few things that have helped but hormonal acne is near impossible to fully get rid of with just skin care products. I use niacinamide from the ordinary, Skin Fix exfoliating pads, peace out serum and salicylic acid cleanser (all from Sephora). Another thing that I think helps a lot is salicylic acid cleanser soap bar from Amazon. It's like $12 and honestly think that has helped me the most. I put the link below. I found it hard to follow because you basically have a really healthy smoothie for breakfast, then salad and a plant based sandwich or salad with a soup for lunch. The supper is like fish, grains, veggies and a salad. Your snacks are veggies.
The neutrogena oil free wash has salicylic not benzoyl in it. For me salicylic has always made my skin drier. Id try benzyol wash and you must give it at least 3 months to work. Panoxyo makes one but this is the exact one i get from pharmacy with my derm prescription. Its creamy and non drying.

Pore clearing by using pressure damages skin if you compress it. "Squeezing" or "popping" is usually going to do some damage to the skin.
The purpose of dermaplaning is to de-cap the blackheads so the hard dry part is gently scraped away without irritating skin, then when you clear out the pore you don't need a lot of pressure. My favorite explanation of dermYouTube. And how to do it gently is by a local aesthetician near where I live, Enilsa. She's popular on Youtube.
I have super tiny blackheads on my chin is that tend to form cysts if the pore isn't cleared out. Dermaplaning is a way to be very gentle on my skin and remove the clogs. If I don't routinely clear out the open comedones, a couple will become big inflamed cysts and leave a nasty scar. When I dermaplaning I just don't get cysts and I can clear out the comedomes without having to use a lot of pressure or hurt my skin.

I’m sure the antibiotics are probably helping. Give it some time. In the meantime, try this rose hip oil: Radha oil from Amazon — the least it will do is moisturize your face. Best case scenario, it cools down the inflammation in the meantime and helps reduce scarring

Leave it alone. You can apply salicylic acid or benzoyl peroxide to help with the inflammation. If it’s above the skin and bright red it may just be trapped blood, if it’s under the skin and feels hard to the touch it may be cystic. You can also get some pimple patches to help with healing.

First of all, I would recommend moisturizing morning and night. As for after the gym, I myself have really oily skin, especially after I get done with the gym. Buy an extra pack of the wipes you use and keep it in your gym bag and wipe your face down real quick when you're done or, if the wipes are too much multiple times a day, buy some gentle facial wipes like these
You should be moisturizing at least twice a day after your treatments! Using so many actives on your skin without moisturizing will cause damage. Also, a moisturizer with SA sounds like a lot paired with retinoid and benzoyl peroxide… Personally I would not combine so many actives, especially not all at once. Of course I’m not the derm, but I would honestly recommend something gentler. Cera ve makes some popular moisturizers that you can get at the drug store. I would say keep trying with the Vichy one but if you find your face still feels tight, look into these alternatives.
Also, if you’re using a retinoid, you need to wear sunscreen every day. Retinoids make your skin much more sensitive to the sun. My fav sunscreen is the Canmake Mermaid Skin gel because it has no white cast. It’s also alcohol free so it’s not drying.

Hi there. So the large one on your chin looks like a cyst that could take a long time to go away on it’s own. I experienced my first one like that last year. Something that might help out with that one in the meantime is the product I will link below. You apply a thick layer to the cyst. I apply it with a q-tip and then cover with a bandaid and leave on over night. This should help it drain. Once you can tell it’s started to drain apply a hydrocolloid bandage to draw out the contents of the cyst. But you can’t reapply the salve once it’s draining since the skin will have an opening.
