We are using Azendoo, is an excellent tool for organising your work with tasks and subtasks, plus its collaborative approach makes it super easy to communicate with my team.
The Interface is super intuitive, i get notified only by the things that concern me so i dont get bothered or distracted by things that dont matter to me.

Of course every elements you mentioned matters. You cannot succeed in a project without having right people at the right place. The process to follow is essential too. But poor communication can ruin everything. if you don't pay attention. So a centralized space within a company to communicate on tasks and projects is essential. Azendoo is the digital tool I found to better collaborate with my colleagues.
Of course it exists! And it's called Azendoo. Actually you've got a time tracking option. This one permits you to plan time you will spend on a tasks. And compare it with the actual time spent. As an example if you plan to spend 3 hours on a task to complete it. This task will display an indication. Those tasks would be displayed as well in your calendar.
Azendoo is the one you need. It has integrations with many softwares as Google Drive or Evernote. You can log-in now as well with your Google account. But the most important is that it's very easy to use and intuitive as well.

Azendoo is such a great investment! It permitted us to raise our productivity like never before! No more meetings or phone calls to follow-up a project! All of those constraints have been replaced by pin-to-conversation feature ;) We're now pinning in each other when we need support. Or if there's a request on a particular element.

Azendoo is a good one. Tasks created can be assigned to any members working within a project. Basically we use it to manage our projects among teams. So far it works pretty well. ;) One feature we didn't try yet is time tracking. It permits you to compare time spent on tasks. You put your estimated time then compare it versus your time spent. At some point we'll give it a try.
One of my colleagues both uses Azendoo and Salesforce (integrated in Azendoo). He combines both project management meanwhile tracking clients. Azendoo permits to assign tasks within projects. And you can benefit from other integrations: google drive, evernote, slack etc.
Slack is effective for direct chats. Since teams encounter different challenges it's useful for direct messages. In fact if you require interactions with teammates Slack requires communication.
Google docs is cool as well. I use it daily with my team. Because we can collaborate around projects.

A friend of mine uses Azendoo for freelance projects with his team. Have you heard of it? He's the manager of his team as well. To collaborate with everyone he assigns a member per task to-do with due dates. Apparently nice and easy to use according to him.
Slack is efficient for direct team communication when I want to get the information directly from someone. Azendoo is my favourite application for collaboration on projects and get things done.
Personally I know what it feels like: lack of motivation, losing tracks of tasks etc... In the past I used to plan stuff to do on paper such as to do lists but it didn't work out. Then after I heard about an application (https://www.azendoo.com/) and it completely changed the way I was organizing myself. I even use it with my colleagues now to collaborate ;) I can create my daily tasks, plan tasks to do in the future and mark them as done when it's over. To be fair I can tell it's very enjoyable because once you finish a task on your daily planner you want to get rid of the other tasks as well! By doing so you become more productive well in my case I can tell it really helped a lot! I hope you'll find your own way to work =)
