Alarm Clock Plus is what I used to use on Android when I needed to get up early.
Here's a random one I googled for iOS
Someone also mentioned Timely Alarm Clock. Given it's been a couple of years since I used such an app, that might be the best suggestion.
Alarm Clock Plus gives you the math option, but with levels of difficulty. You also can set it to require you answer a few equations instead of just one. If you're on Android it might be worth a try.

No issues with the AOD and using Sleep as Android as my alarm clock. It shows up fine.
EDIT: Alarm Clock Plus hasn't been updated in almost 5 years. It was abandoned before AOD was a thing, so I don't think it's the AOD's fault at this point. It just wasn't designed to work with an AOD
On notification sound, it seems to me just certain apps. I have a lot of apps that respect the default sound, but it seems certain devs just did their stuff to ignore that, like Facebook for example who also adds their own notification sound.
No experience on mine that I noticed regarding apps stopping normal except for one app (It's a rewards app from my carrier that hasn't been updated in 6mos). I was actually surprised when I woke up later and fired up Alto's Odyssey and found that I had pressed home prior to sleeping and it was still paused in mid-jump.
With the Alarm Clock Plus app for Android you can set the difficulty of the math problems. At the simplest setting it's basic addition with no carrying like: 3+2; 4+4; etc.. I use the next level up with often includes answers over 10: 6+8; 4+7; 5+9. These are simple when wide awake, but when I'm half asleep they take a minute.
AND you can set how many problems you have to get right in a row to dismiss the alarm. I need to do 4 sums in a row without mistakes to shut it off. If I'm really groggy, it takes a few tries before I'm alert enough to do it, and by that point I realize why the alarm is going off and don't go back to sleep without realizing.

Here's what I use to make sure I take my meds no matter how distracted I am (otherwise I'm HORRIBLE at taking meds on time).
Oh, and Seroquel has given me 90 lbs since I started taking it. And the horrible dreams were rare and lately they've been more regular. /sigh - but hey, I can go to work so there's that, right?
Thanks a lot! Have been using Alarm Clock Plus for many years.. Tried alot of alarm apps. Maybe your app will end the search for a replacement
O used to use this https://play.google.com/store/apps/details?id=com.vp.alarmClockPlusDock
I don't remember if you could choose thw soumdof the chimes, but i do remember that you could set chimes
I use Alarm Clock Plus and since the last charge GSAM battery monitor doesn't show any measurable battery drain for the app.
I've been using Alarm Clock Plus for years. You can set as many alarms as you like with either stock android sounds or your own music, alarms can be configured to start gently or at any volume level you like and there are multiple snooze options.
I don't think i've ever hit snooze more than twice with the stock app on my M9, but I did have that issue on my M8. I ended up using a different alarm app and everything worked fine after that.
Here's the one I went to: it's served me well for many years. https://play.google.com/store/apps/details?id=com.vp.alarmClockPlusDock&hl=en
I found this app to be particularly useful. Alarm Clock Plus Dock https://play.google.com/store/apps/details?id=com.vp.alarmClockPlusDock I set four recurring alarms for any given morning. First one is soft and sweet using an acoustic guitar didly I recorded awhile back that requires only a quick press of a large and friendly 'Dismiss' button. The second is called old faithful and utilizes a pretty stereotypical nightmarishly loud repetitive screech/siren/beep that requires something special. This is the key to the app's success; you must solve 3 (albeit simple) addition problems to turn the alarm off... Doesn't seem like much, but when you are in mild panic mode trying to turn the stupid noise maker off AND simultaneously trying to think through simple math, you're awake pretty quick. Unless I'm working on <4 sleep (and I hope you get plenty as that's uber-conducive to a fresh start) I always become totally conscious and act on my conscious brain after the math. Might be worth a try... Sorry if you haz an iPhonz. Prolly something similar in the iChurch.
I set it to give me math problems in the morning so it helps me wake up. I can also choose the difficulty of the math. Currently it's on "Medium" so I get a 3 number +/- problem. Usually in this type of formula: ## - ## + ##
The alarm won't turn off unless I answer a certain number of problems correctly, which I can set (currently at 3). I can skip a bunch of problems until I get to an easier one (which I know, defeats the purpose, but at some point, it gets annoying to have to carry ones in my head!)
This.
When I tried it, Sleep as Android highest difficulty was too easy, I use Alarm Clock Plus★.
Also, I'm getting better at simple arithmetic now and the easier difficulties I can do half asleep.
Alarm clock plus https://play.google.com/store/apps/details?id=com.vp.alarmClockPlusDock android version
I like Alarm Clock Plus because it has a huge snooze button.
Not gonna search, but my Android has these "lesser known" apps:
Alarm Clock Plus -- This is the app that can require math to turn off
Business Calendar Pro & Business Tasks -- keeps me on track
Dosecast -- I have memory problems, and helps remind me to take my meds
Droid48 -- best RPN calculator I've found
Extensive Notes -- note taking app. used to write college papers (first draft) on the bus. Full disclosure, creator is a friend, and I get paid version for free.
FBReader -- can handle epubs, html and txt. Haven't tried other formats.
Grocery IQ -- best grocery app. Ever.
IwantIM -- not my first choice, but Pidgin isn't available. let's me connect my msn, icq, yahoo and jabber accounts all in one place.
LLama -- open source Tasker.
Montana -- love this game.
Screen Filter -- why your phone so bright?
Finally, I don't have a vehicle, so I removed it, but ACar.
Edit: Totally forgot Hackers Keyboard. Has just about every language/layout, including Dvorak, and gives you a full keyboard.

What I did was I put my alarm on the other side of the room (and made sure it made the most obnoxious sound I could find, the Red Alert klaxon from Star Trek though the biohazard alarm works nicely too) and use an app called Alarm Clock Plus turn off snooze and set "Math for Dismiss" with 3 problems (you can set the number of problems and difficulty). If you want it to shut up you have to get up, walk across the room, and do math and by that point I'm pretty awake. Guy I used to date in HS, his dad set 3 alarm clocks, one in each corner so he had to get up and do a full circuit of the room to shut them all off. There is also Clocky, Tocky, This flying thing, A shake weight alarm clock, and this Bluetooth bed shaker
Though in all seriousness you might need to drop one of those internships if you are this exhausted that you are missing class.
This one has lots of other great features too...

I use this one: https://play.google.com/store/apps/details?id=com.vp.alarmClockPlusDock&hl=en
Pretty happy with it.
Also, it's hard to confuse with the calculator.
I need things to motivate me. I usually have so much (too much) going on that I have a reason to get up. I don't mean to offend, but it feels like you aren't looking forward to anything.
Other things I use:
Alarm Clock Plus - I love math, and this app forces me to solve math problems to snooze/dismiss.
Your post-it note analogy, I completely understand. My wife and I are in a days long fight right now because I wouldn't pick clothes up around the house. I take them off where I am, and barely notice them afterwards. To her, they are a steaming pile of poo that is extremely out of place and needs to be taken care of NOW. (Sorry, rant, but I am getting there). Things fade to the background easily for me.
To avoid that, set a reminder, repeating daily, for a time before you brush your teeth. When it goes off, snooze it every 5 minutes until you brush your teeth. If you forget, go back to the bathroom the first time you see it, and floss. THEN dismiss the reminder. The next day, there will be another!
Hopefully some of this helps. Good luck.
EDIT:
One more app I use (Android, don't have any suggestions for iOS) is:
I started using this because I was cheap, but its limitations have actually helped me.
The free version has a 6 hour max snooze. The full version is $5. I would have reminders come up, and snooze them, and snooze them, and snooze them. I still have 2 reminders on there from months ago, and see them 3-4 times each day. They are projects that I haven't gotten to yet.
I didn't buy it, initially, because I was cheap. When I went to buy it, I actually read reviews, and it seems the developer has stopped supporting it, as of a couple of years ago, and people are having trouble with it. So I stuck with the free version.
OK, now I'm done.
I use alarm clock plus, for android
