You can controll all volume settings individually, and you can store 5 different profiles and add them as a widget (the size of a normal icon) on your screen. It is perfect.

You can just your combo of 6 (media, ringer, alarm, etc.), but I only adjust media.
Fine volume control app. I use it for podcasts and it works perfect.
There's precise volume as well which is material design but its too finicky for me.

I haven't needed one, but a quick search yields this among many others. Support seems to vary by app/hardware so maybe try a few of the trials and free ones to see if one works.
OK phone has arrived and yes it is annoying as hell.
This app adds more steps but it also jumps around when you hit 100%, hopefully there is some way of sorting this without rooting.

Yeah. That "first increment" you're hearing is almost eardrum busting in respect to headphones. It's pretty messed up.
This app has helped manage, though I can't say if it's really worth much to a Bluetooth speaker kind of thing.

Apparently, for Android you will need to edit the OS yourself (you need to be rooted) ~~or use a custom rom~~ there doesn't appear to be any.
For Windows, there may be an app available.
On Linux, it depends on your distro; it appears Ubuntu had a value-step key but it was removed due to being buggy.

Only app that gets the volume low enough. Looks like crap though.

Adding more volume steps (or granularity) is largely hit or miss. You have to try various apps to find one that works for you.

I don't believe so. Try Fine Volume. It's pretty old, and I've never tried it myself, but it claims to do this.
Surely it looks bad but the last time I tried it, it did seem to work.
On Android you can try: Fine Volume Control. Don't know if anything exists like that on iOS.
Fine Volume Control app will let you access lower volume levels.
