For my purposes the main advantage was that I can have my ears open at all times and I can use my stethoscope. Another advantage for me which is somewhat unrelated is that I can use Out Loud to read my text messages out loud to me throughout the shift. At the end of my shift, the fact that I have eliminated (for the most part) the need to turn my phone display on has resulted in massive battery gains even though I have NFC and Bluetooth on at all times. Also, because I use Project Fi, I am always on WiFi, which has eliminated the cell radio battery drain.
But yes, the bone conduction headset was absolutely a game changer for my purposes, although I will say I feel that much of the "bone conduction" that is advertised is just the speaker proximity to the ear, and not so much actual bone conduction, which makes sense, since the waves have to bypass lots of tissue and fat to make it to the inner ear.
I use Out Loud, though my car doesn't have bluetooth, just aux. I have it set to detect when I am driving via accelerometer and reads any notifications I receive from Hangouts (as I use that for SMS) and it works every time.
I use an Out Loud profile to read out selected app notifications in the car over the media stream. Play store NB: there is no facility to reply with voice.

Out Loud is what you are looking for. Has a lot of customization settings (turn on when BT connected, different languages, blocked words, etc) and is very easy to set up.
I've done exactly what you want to achieve. Only difference is on my phone not my Google hub. No reason this couldn't be done on a cheap Android tablet.

If you are using android, this app can be setup to have your phone do that for you (along with all other notifications). And it can automatically turn on and off only when connected to a specific bluetooth device (or headphones plugged in, or a number of other things).
I've used it for years and love it.
Out Loud - Uses TTS to read notifications. You can enable it just when headphones are connected. I use it for getting notifications while I'm running (or walking). It's not perfect, so any recommendations for similar functionality would be appreciated.
Have you tried Out Loud? I use it to read messages automatically when I'm connected to my car stereo - no need to tell it to read it to me. Only, responding isn't really an easy option...
But yes, I agree this needs to be a native feature. I wonder if there's some Apple patent on the idea... Seems to be the reason some obvious features are still missing.

Why do you need a notification sound to tell you about the nature of the intrusion when you could add that info to the notification message itself? This is assuming you can use the Out Loud or SpeakMe or Notification announcer apps to automatically read each notification from specific apps.
