I am confused of the first two drug formulation. What about Keletra which is lopinavir and ritonavir. LPVr+ Atazanavir + cobicistat, or ATV/COBI (Evotaz) or what is called fixed dose medications Like Truvada and Combivir and trivir.

Whenever I was traveling with family, I used to carry one of these and just kept it in my suitcase. Fortunately I don't have any family members that would pay much attention to it. I could just grab it, go into the bathroom at night when I brush my teeth, then take my meds, and put it back in my suitcase.
If you can use something like that to carry them in, then I'd stick with a reminder on your phone or watch (a repeating 24 hour one), that might work for you. The modern day meds are a lot more forgiving with variance in the time you take them. My doctor said as long as I take them within a 6 hour period, I'm good. I test that out often, and haven't had any negative effects in the past 30 years.

Buy a pill carrier for your keys. They are small and discrete and cost very cheap on Amazon. I put my pill in every night (set a reminder) and take it in the morning. Then you dont need to do anything to remind yourself you can just shake your keys to know you took it.

A weekly pill dispenser is a good idea, but if you're only taking one or two pills a monthly dispenser like this (link) is more convenient imo
24 just passed a year on Biktarvy. Side effects post was because my timing was off by 3-5 hours per day
I take mine at 4pm Because I still have food in my stomach from lunch and that’s enough to pad the medication as I call it
The closer I take it to the same time every day the less stomach aches and other weirdness I have.

I take mine within about a 4 hour block every day. Alarm set for 7pm that reminds me every half an hour, around dinner so I can take it with my meal. I use Lady Pill Reminder for Android 😂 it's easy and simple. Sometimes I take it at 5, sometimes at 11 or 12, but usually between 7 and 9. I try to be diligent about taking it right when my alarm goes off, even if it just means eating a few chips or a chocolate. My Dr said even if I just grab a drink of something besides water to activate my stomach acids I should be good.
It's not like birth control that regulates your hormone cycles very precisely, these drugs stay active throughout your system for a couple days. I think of it like topping up your gas tank before it gets close to empty.
I haven't had problems with keeping myself undetectable in these 5 years with this routine on Genvoya, but this is purely anecdotal. The doctors say take it regularly because that is what was studied. It would probably be unethical to tell patients to take the meds inconsistently just to study the effects

You could get one of these. That would allow you to discreetly bring your pills with you everywhere and then you can just discreetly administer your pill. If someone does happen to see you medicating, hopefully they're tactful enough not to say anything but if they do you can politely tell them that's a weirdly inappropriate line of inquiry.
