I believe it is the widget from the Ocea widget pack. There is a widget in there called AndroMono, but it doesn't look like what is shown in the image. /u/AshVasquez I must know how you got that widget; I love it.

Both the nav bar and notification bar are hidden through adb commands. This way the screen is in infinity mode practically all the time. I also use SystemUI Tuner to hide everything except the Wifi icon and mobile data icon on the notification bar when it is being shown.
Found it! It's about 2/3 of the way down the 'Kustom' tab in Ocea :)

First, you should get nova launcher and set up kwlp and kwgt. Play around with those for a while and see if you can come up with anything. An easy way to get into making widgets is to use premade widgets in packs such as ocea for kwgt and sort of deconstruct them to see how they work. Icon packs are also a great way to personalize your homescreen, here are a few of my favorites:
