We are running a full-pvp/full-loot/firstperson/sandbox mmo with no quests. It's.. like a cross between skyrim and old UO.
It's very niche and I know a lot of folks who just hate it but then there's the people who REALLY enjoy it and can't go back to anything else.
It's f2p but f2p char have their skills capped so it's more of a infinite trial. Thou if anyone want their first month free send me a PM and I will hook you up with a code that gives new acc a month of sub!

Playing Perpetuum right now. Its like EVE Online but without the monthly subscription, and instead of ships in space you're mechs on an alien planet, otherwise its the same concept.

I'm a sucker for games that allow me to craft and manipulate the market. If I don't have to fire a shot to succeed then I'm content, I mean I'm not against violence but I'd rather have someone else die for me then go out and die myself.
As for upcoming MMOs I'm quite hyped for Fortnite currently, it looks to be right up my alley with crafting, tons of loot, base building, etc.

I recommend you go look at Salem The Game. The graphics are a bit odd. But its a good game that aside, you have full loot pvp, there is permadeath however to actually kill another player is a MASSIVE undertaking as it requires having learned all the skills and means opening yourself up to retaliation so people outright murdering one another is usually a rare happening.
Gameplay is that you're a colonist from england sent to colonize the new world, sadly they've picked the very worst person possible as you know nothing, literally. You have to forage food and natural goods to research and improve your understanding of your surroundings, the more you research the more buildings you unlock, the more stuff you can craft, ect.
Well 2 really niche games are Salem and Heaven and Hearth. They are both mmorpgs, they have TONS of crafting/building. They have however permadeath and for that reason you will die now and then and have to restart you character (or play with one of your alts). I feel you, I also love muskets. Risen 2: Dark waters and Risen 3: Titan Lords both have muskets in them, and using musketeer is more than viable build in them. Both are pretty good RPGs, with IMO good graphics. They were developed by Piranha Bytes though, so expect some bugs and wonky combat mechanics. Also http://www.salemthegame.com/ Salem has muskets in it, but it's a completely different type of a game (open world top-down view survival set in new world colonization period, tons of content but seems to be dying)

But these are quite grindy. Both are initially from the same devs, but Salem is run by some other guys at the moment.

Very deep crafting, player driven economy and towns, you need to harvest in order to survive, make your house anywhere, politics and stuff like that, very sandboxy and player driven.

You should check Salem. It's still in beta but has most of the features you mentioned. The only downside is that it's more crafting/building oriented. It has PvP and perma death but the combat mechanics are pretty cumbersome.

Pretty cool game concept. I felt the gameplay centered way too much on finding and crafting curiosities to research. From what I understand, Salem is a similar game.
Edit: it looks like H&H got a 3D overhaul since I last played.
One of the most competitive survival game I have seen is Salem the Game at http://www.salemthegame.com/ . it can be played casually too. If you are into guns and stuff look into arma 2 or 3 if you have money. Arma 2 has dayz mod that is like H1Z1. If you are into combat, Mount and Blade: Warband multiplayer is absolutely addicting.
F2p not, but Minecraft or most voxel based Games are much like wurm online, or "life is feudal your own" but this is not f2p too, they are all mostly buy2play. The only free to play game I can think off is Salem: http://www.salemthegame.com but I can't say how good it is
Salem and it's predecessor Haven & Hearth although they have a fighting system (pretty weird one too) fighting is mainly used for hunting animals for resources and the games rely more on gathering crafting and building in a sandbox world
So this game probably won't be out for another year or two but EverQuest Next sounds ideal for you. It will be a sandbox game with a destructible environment and permanent change. It's also supposed to be the successor to the original EverQuest games and I see that you've played those. The game would be out by now but they've had quite a few hiccups and setbacks. As of a month ago though they've officially shifted focus to EQN so we should start seeing more from that game soon.
Other than that there isn't much to play now that is similar to EQ, FF11, etc. Albion Online will be out early 2016 so that might be worth looking forward to as well.
Edit: I did a little digging and I did come across Salem which is supposed to be a sandbox MMORPG. You could give that one a try.
The Elder Scrolls Online is definitely what you want. Quests are actually interesting so it makes you feel important going through them. It also has the whole Elder Scrolls world to explore from Morrowind to Hammerfell. Skill is also involved because the combat is like the other Elder Scrolls games if you've played those. You're going to have to actually aim your attacks and spells.
There's also Wildstar which isn't the most difficult game but I've heard their dungeons and raids are some of the most difficult out there today.
