Nova Launcher Settings:
Being inspired by another theme and downloading the theme then changing some things are two completely different things. Switch UI. If you did change anything, it was minor.

Edit: ~~The only ones it looks like you created yourself were the first three.~~ I'm not even sure about that though, I'm just going based on the images I can find of Switch UI on the internet. The last 5 are a direct rip from the theme.

I'm using Nova Launcher and the Switch UI Icon Pack which has square (with rounded corners) icons. It's paid but if you uninstall it within 2 hours it auto-refunds you.
Another thing I really like about Nova Launcher is that if you long press on an icon, and hit edit, you can choose what icon to use for it from any icon packs you have installed.
Using Nova Launcher 4, the icon pack is Switch Ui, my icon size in Nova is at 85%, and my grid is 9x7. Hopefully that answers all those questions.

I don't remember where I've download the wallpaper from, it's probably an iPhone 6 default one.
My poor N5 died from the constantly rebooting power button, so I just joined the 5X family. Got the 32gb in white, and it's great. Feels way better in the hand than the huge 6P.

Updated my previous set-up. I cut the number of icons in half and changed to a very dark background (depending on your monitor it might look almost completely black - but it isn't) to further simplify things. Added a two week calendar for equal parts looks and function.
