Don't panic, that is totally normal! When your milk first comes in your breast tissue gets incredibly swollen because it just isn't used to being full of milk. Most of that swelling is actually just from your breast tissue being inflamed, not from milk. That's why expressing milk only gives SOME relief, not complete relief. The swelling will go down over the next few days.
As for helping your nipple stick out more so your baby can latch, there's a couple of things you can do. You can use a breast pump(manual or auto) for a minute, that should pop your nipple out.

I thought it was because you can’t add warm milk to cold milk. It was my understanding you can combine them once the fresh milk is brought to be same cooled temperature.
Fellow large breasted mom here. I highly recommend the side lying position as it’s very comfortable for both you and baby and can ease the flow of milk. Also, until baby gets better head control and when side lying isn’t an option, you may want to invest in the “my breast friend” nursing pillow. It provides excellent support so you’re not straining your back while nursing. Both of those things were absolutely instrumental in kicking off a successful breastfeeding relationship with both my babies.

Avent also has storage cups that are like tupperware that are compatible with the Spectra--you can pump straight into it, throw on the cap, and then throw on a bottle nipple if you don't want to transfer.
Those are very safe, memorable guidelines, but in reality there's more leeway than that. The Mayo Clinic article linked in an earlier comment has a secion in the middle of the post detailing how long breast milk is safe when stored in various ways. KellyMom also has a lot of information about feeding and storage and is a common reference.

At 3, your daughter understands a lot - perhaps explaining no more milk, but cuddles anytime are available. Alternatively, I’ve seen success stories of using bandaids and saying mommy has bobos but echoing the other poster: you’re wife will need the willpower to enforce no more nursing. There will be tears regardless as your daughter will be denied something she wants but you can both comfort her in other ways.

I couldn't wear anything the first 6 weeks. A light breeze would make my nipples scream in pain much less a bra or nursing pad. DH and I fought more about the house temperature than the baby those first few weeks (LPT: If you wanna die an early death tell your postpartum wife to just put a shirt on if she's cold.) If you decide to go the route of discreet pumping in public area, I would suggest using [https://www.amazon.com/Freemie-Collection-Cups-Concealable-Funnels/dp/B00UV6JDN8](Freemie) cups and quiet battery operated electric pump. The freemies can be worn under a loose fitting shirt with just a couple of thin tubes going to the pump.
Keep the breastmilk. You don't need a fridge, an ice pack will keep it cool.
I pump at my desk, which is in a room with 20 other desks and no partitions. I also pump in the hallway lounge during my 4 hour class, since I'm already missing class to pump, and I'm not going to add extra time scurrying to and from the lactation room. At least here, "public indecency" has to come with lewd intentions, and pumping milk has a obvious, non-lewd purpose.
I use a cover (it's cow print!). Nobody in my office has ever said anything negative, and people either act normal or just ignore it (I had a friend make a point of chatting with me the first time I did it so that people wouldn't think I was off-limits).
Pumping at my desk is a game-changer, honestly. It would take me at least twice as long to pump if I had to get up and take my things across campus every 2 hours. And it helps me to feel "normal"--I talked to another woman in my program who complained that she had a hard time making friends when she entered, since she was constantly running off to pump and missing the friendly chatter in the office.
I survive breastfeeding the first couple months with the help of this.

We used them upside down (like instead of using your thumb to do the cut lever thing have your forefinger & hold your thumb steady. I also cut each nail three times - side other side & then top to have it be more level.
As for pumping I reccommend a haaka you can catch your milk from the side he isn’t feeding off of. & yes he may eat from both sides, but if you do this you’ll be giving him more hind milk & he might sleep a little more.
You can also pump before going to bed. That way a big producer. Making sure I was completely empty before going to bed & then waking up & pumping after the feed to completely empty again.

Are you going to be home with baby? If so, don’t worry about pumping daily because you might induce an over supply. As someone else said, the hakaa can catch let down from the opposite side (milk that might just leak anyway). I also like the Milkies Milk Saver to collect let down. I store the collected milk into 1 ounce sticks using the milk tray.

We use Baby D Drops brand (amazon link), you just need one drop instead of 1mL syringe. I usually just put a drop on my finger and rub it on LO's gum line, but sometimes I'll apply to my nipple before breastfeeding.
Here is a really good example of how a flange should fit. You should have about 1mm or 2 of space left before hitting the side of the flange.
If you are going to buy new sized flanges I would highly recommend getting a set of pumpin pals. They replace your flanges and are angled and molded more comfortably. You can get a set of 3 sizes on Amazon Here and they actually made pumping comfortable! They fit right into any Medela connectors.
You are doing a great job! You and baby are both new to this and it takes time. Remind yourself that any decision you make is the right decision for you and baby at that moment.
My son refused to nurse for a couple of days after leaving the hospital because of a lip and tongue tie that we had fixed. I started pumping and bottle feeding, which I felt really guilty about. A lactation consultant made me feel better about it and gave me a plan. We continued to pump and bottle feed but offered breast first (except at night, sleep is important). We kept him on a 2 hour schedule. If we waited for him to show feeding cues, he would start screaming and was so hard to calm down. An angry baby just isn't going to latch. When baby was calm or sleeping, I would put him skin to skin and offer breast any time he showed feeding cues. Eventually we got to a point where we were only breastfeeding and I was able to stop pumping.
I was also given a book by a lactation consultant and I loved it! Its not super long like most and it was really helpful. As for the crying and fussiness, we are still trying to figure that out at 6 weeks old. Some nights he doesn't sleep more than 20 minutes at a time. But everyone keeps saying it gets better soon, so here's hoping!

You can leave the padding in for some more coverage or take it out for more flexibility in pulling the cup down. I've been wearing the same set of 3 since I was pregnant and they're holding up well.
Nursing tanks were also great especially in the beginning. Target has some decent ones, but I didn't love them, they were cut a bit narrow at the top for me. Nobody really wants nursing boob side boob lol.
I also experimented with the one up one down method. Basically you layer a tank under your shirt. Shirt goes up, tank goes down. You can make it a one stop shop with a nursing tank or just use a loose cami that can slide down or even just live under your boobs. This way your stomach is covered. Baby's head will cover most of the exposed boob as well.
My babe was born in June in the south so it was super hot to be wearing two shirts all the time. Most often, I would use a cover or a muslin and just hike my shirt up, unclip my bra and be half naked under the cover.

The cooler I linked fits perfectly in the left hand compartment. Then when my bag is open, my pump fits in on the right hand side. My spectra 2 and my medela pump in style have fit. Then I put my pump parts ontop of the cooler but in the main compartment. I can PM you a picture of it all packed up.
This is so tough. I went through the same thing. My lactation consultant put me on a triple feeding schedule (similar to what you’re doing)... feed max of 10 mins on each side (20 total), supplement with bottle of pumped milk or formula, and pump. Works best if a spouse or someone else can bottle feed while you pump.
Pro tips for the pumping: keep parts in the fridge between sessions. I put the flanges in a bowl, empty the collection bottles into a feeding bottle, then rehook the collection bottles to the valve part and put everything in the fridge. I also wear this kindred bravely bra which can be used for nursing AND pumping. Saves so much time! It’s a little pricey, but definitely worth it.

I think your little one is at the perfect age for early intervention. Honestly, it's free, they come to your house, and if he doesn't qualify it's not a big deal. We get a lot of referrals around 15 months, and it's usually the age that a speech therapist will start to see a kiddo. There are definitely other factors (like genetics) that can affect when babies start talking, but there's a lot of evidence that early intervention is beneficial. The evaluation process can be a little lengthy as week, so better to start sooner rather than later.
The drinking is definitely something to bring up during the evaluation- if he does qualify for services a speech or occupational therapist could take a look and make sure there's not any problems with motor strength or planning. You could try doing a yogurt kind of drink in the straw cup- it's more interesting than water and the thicker consistency is easier to control. Also, make sure you're using a real straw cup like this ( https://www.amazon.com/dp/B00G6EE6NG/ref=cm_sw_r_awd_pjxyub12Q8K43 ) and not one that requires biting the straw (those are more complicated and not worth the fuss).

Spit up can definitely have some force behind it, especially if it’s with a burp escaping.
Some babies just spit up a lot. My son is one of them- he spits up pretty much after every feeding. Doctor said it’s not a problem as long as he isn’t upset by it.

I almost exclusively pump. Plus, I’m a slight over-producer so I have a good back stock and I actively donate my supply. From this I’ve learned a lot of milk storage lessons.
I find 7 ounces works well for me since my little one tends to take bottles 3.5 ounces at a time. So this works best if you’re always freezing about the same amount per bag.

I have 3 milk bags with 7 ounces of frozen water each for weight and balance. Two are on the tray on top and one goes on the bottom tray to help balance and guide the milk bag to freeze properly. I fold the tops of my milk bags under so that it doesn’t stick out and form sharper corners, plus it makes it more compact.
Once they’re frozen, I store them in a gallon ziplock bag. These I use a sharpie to mark with date range to best help rotation of stock. I also color-code my dates by month. May was a dark pink, June is purple, July will likely be blue. Makes it even easier to know if you’re using oldest by sight! I mark the milk bags with the same color when labeling them.
Also, if you have a drawer freezer with space, drawer dividers can help you organize milk for use

I read a couple books as a FTM and one I'd recommend for your SIL is Latch: A handbook for breastfeeding with confidence at every stage (Amazon link). Practical info on how to breastfeed and common issues and how to address with a few short examples of other mom's breastfeeding experiences. Supportive but not pushy in tone.
Water and oatmeal. And more water.
Have snacks handy wherever you plan to nurse overnight too!

First: do you have a haakaa? I would get one ASAP if not. It will pull your milk out passively so that you don’t accidentally create an oversupply with the pump. If you need to know how to use one, happy to ELI5. (Get the smaller one - here) but target and other places have them.
Yay! Also, I’m just here to say that if that’s an iron supplement for baby back there I highly recommend the version I found on Amazon. It’s about the same price and it doesn’t make my baby barf disgusting smelling brown stuff all day! Lol. Just in case anyone else has the same problem!
I recommend reading up on the safe sleep seven or even the book sweet sleep so you can make informed choices and bedshare as safely as possible.
We have bedshared from the beginning and continue to because it just work for all of us (kiddo is 18 months). We have gone through different phases - kiddo on my side of the bed with a side car crib, kiddo in the middle of the bed with only me and husband elsewhere, and now as a toddler, everyone in the bed together.
Bed sharing has made night nursing so much easier and has definitely made it so that baby and I wake together - he doesn’t rip me from a deep sleep, which makes waking so much easier on me.
Cuddle curls is definitely important. You want baby far away from any entrapment or suffocation hazards like pillows (make sure it’s a single firm one) or blankets (light weight to your waist). Mattress has to be firm enough too.
Another important aspect I didn’t realize at first and just got right by stupid luck is that baby is supposed to be unswaddled - they need to be able to kick their arms and legs around freely. Sleep sack is ok.
I legitimately wear one of these 90% of the time I'm at home.

The best ones I've used are the Johnson contour pads because they are shaped to fit inside my bra, they aren't too huge and they are soft. I've only found them at one store near me, Walmart I think, but now I've found them on Amazon so I'll probably start buying them there once I'm done with this box of Lasinoh.
I had a hand in getting our room ready, since I will be the first person using it when I come go back to work in December. We got the following sign from amazon. I'd also recommend setting the room up in outlook to block off time, especially if there's going to be more than one pumping mom. It helps you all of you can stick to a schedule.

It might be a good idea to make sure your shields are sized correctly. My LC recommended Pumpin Pals (Amazon link ), which come in a set of different sizes with the idea that you use the biggest when you're most engorged and the smaller sizes when you're less so. I love my Spectra -- well as much as one can love a machine that sucks your nipples. I started pumping in the hospital with a Medela Symphony, and I get so much more milk with the Spectra.
I exclusively pumped for several weeks while LO was getting the hang of latching, so here are some tips:
I'd recommend getting a power invertor so you can pump in the car if you ever need to. I have this one from Amazon and it works great.
Prepare a "pump bag" to hold all your supplies -- hands-free bra (I have the Simple Wishes bra), pump, storage bags and bottles, permanent marker to label storage bags, a little container of olive oil to lube up my flanges (helps to prevent rubbing), small kitchen rags to wipe up milk, extra pump parts, etc. I use an LL Bean tote bag to transport everything.
In between pumps, I store all my pump parts in a gallon ziploc bag and stick it in the fridge so I don't have to wash them after every pump.
I got a couple of those silicone suction cup "pumps" and I'd slap one on whichever side baby wasn't currently nursing. It was just enough to take the pressure off, but not actually pumping and increasing supply. I slowly built up a teeny freezer stash that way, too. Now, 6 months out, I throw one in my purse if we go out on a date night or something just in case I start feeling too full. Somewhere around $12 on Amazon.

Honestly, I hadn't even thought about nursing bras until right before I gave birth when someone mentioned it to me. You could pull up, sure, but I find the nursing bras to be a little softer on my chest (which can get really irritated those first few weeks).
I had gotten in the best shape of my life pre-pregnancy so my boobs were small but now they are HUGE. I went from B to DD. Of the two nursing bras I bought, this is the one I wear practically every day. It's more comfortable and offers enough support, plus each "size" is a range of sizes so it's got a little stretch.

I found a pumping bra here on Amazon that just clips over a nursing bra so that I dont have to take my shirt and bra off. I've found it to be very convenient for work.
Button down tops with a camisole underneath. The camisole can be pulled down from the top and the button down can give extra coverage. I also got 2 nursing shirts off Amazon that are casual that are my go to t-shirts. They are a little loose so good for postpartum, but not intended for maternity so I’m not walking around in a sack.
I also wear a true wrap dress which allows the bust area to be pulled far enough to stretch.
I would encourage you to look up the French concept of the capsule wardrobe. Basically having 10 or so interchangeable items. It helps you to feel like you always have something to wear even if you only invest in a few pieces.

After dealing with poor support from super stretchy, Lycra nursing bras and then getting thrush, I wanted a supportive, cotton nursing bra. I bought eight of these because they’re so inexpensive compared to most other nursing bras I’ve found and I love them. They don’t ride up in the back, they’re supportive and fit well (which is handy in general but especially when I was engorged as I started weaning a month ago, or when I was traveling for work and couldn’t pump as often).
For sizing, I am 5’5” and 160ish lbs (😳), and I wore a 38D before I was pregnant. Once my supply adjusted, I was just slightly fuller and the XL fits me very comfortably. I’ve worn this brand exclusively for nine months. Luckily, Amazon has a wonderful return policy too if you order the wrong size.

I've been staunchly against cosleeping but I have been sick for 3 weeks with a virus that is like a cold but it is also in my eyes. I have light sensitivity and have to lie down in the dark a lot. (I'm communicating with my doctor regularly.)
I have been nursing in bed during some of these times because DD is also sick with a shotty nose and has been nursing a lot. I fell asleep once with her latched and woke up to my husband trying to pick her up and put her in her crib without waking me. It terrified me. She is 16 weeks and could roll over, but I could roll onto her! It happened to my husband's cousin, she fell asleep on th couch with her baby and the baby fell into the crack in the cushions and suffocated. It is so sad.
I use a cosleeper with mesh sides and it helps immensely. I only sit up to breastfeed now (No lying down unless I'm not a bit tired), and the cosleeper makes it easier so I can bedshare safely on occasion. I never have her in the bed when my husband is in the bed because he is a heavy sleeper, a sleepwalker, and be flails his arms. It is difficult to resist letting my daughter sleep next to me as it is comforting for both of us, but I am too afraid something would happen. I got the cosleeper on Amazon for like 50 bucks and we keep it folded up under the bed so I can grab it when I need it. I highly recommend one. I feel it is the only safe way to bedshare.
Anyone who gets this cosleeper, I recommend you use a small fitted bassinet sheet to cover the mattress (if it fits tightly), because the mattress cover that comes with this one is polyester and I noticed it rubs off my baby's hair. Reviews on Amazon said it too. Happy safe sleeping, everyone!

I found these to be very useful, both in hot and cold versions. I actually bought 2 sets, one to keep in the freezer and one that I could heat up if necessary. Now that I'm 4 months out, I mostly use them for heat because I have Reynaud's, and that affects my nipples.
Your supply should balance out soon, but so much you just bring too much to the party. These are the only thing I will use.

My lactation consultant said the best thing you can do is to express some breastmilk and rub it in to your nipples, let it air dry, and wear breast shells between feedings to let your nipples heal.

I have long been citing the Lancet study which showed

Which means that a woman who has 3 children and breastfeeds them each for 1 year would have a greater risk reduction than a woman who has 1 child and breastfeeds that child for 3 years, due to the extra risk reduction of pregnancy. But in general, for whatever kids you have, nursing longer further decreases your risk.
My baby started feeding s lot more quickly and less times a day around 3 months exactly. Longer than most people said it would take, (most say around 2 months) but we got there. She’s a large baby so idk if they had something to do with it? She fed like 10-13 times a day until 3 months when she started feeding more like 7-10 times a day. Around that time I stopped having to feed from both breasts too, I only feed one breast each feeding, except if she won’t go down for bed or a nap I’ll switch to try to make her sleepy. She’s 5 months now and able to feed in like 5 minutes if it’s not a “nap feeding”
However my breasts are STILL leaking at over 5 months pp. I still wake up with soaked shirts/bras, and definitely need nursing pads constantly. My breasts don’t get rock hard as often, only maybe once a day before bed or in the early morning if she hadn’t fed for a while. They’re usually a lot softer even if they’re full. I bought these reusable nursing pads about a month in and I don’t regret it at all. Lasinoh makes a pillow you can stick your arm into, maybe try that because it’s smaller and more portable than a Boppy? I also wonder if there’s a nursing bra that will help support your breast better. But you could always bring a rag/towel with you and roll it up under your breast so you don’t have to hold it (or maybe stick it into your nursing bra).

You could also try grapefruit seed extract in pill form in combination with the ointment. It is a more homeopathic option, but helped me, along with cutting sugar and hot washing everything with vinegar in the rinse cycle. Sure thing! I also found it while researching how to feed while baby wearing (I knew my older kiddo would not stand for me being stationary all day prior to baby getting here). I watched a ton of YouTube videos, got one, practiced with a teddy bear before baby, and got comfortable just wearing it properly first. Then put lo in. Just make sure the fabric isn't twisted in the rings and it's pretty easy to adjust, and that there is a good "seat". You Tube videos will explain all of this too. Here's a link to my favorite one I have (it even has a pocket)!
There are cheaper ones, but this one is my favorite because of the quality of the fabric, the pocket, and ability to be breathable in hot weather but warm in cooler weather.
Edit to add Volkup is also a solid brand that's good quality! I have one of theirs too and they have more cost effective slings made from lighter weight material.

I feel like lactation consultants are underrated once you get past the early days. I recently saw one (8mo in) and she had asked a bunch of questions and gave me some good advice. I had read the biting page on Kellymom at least five times without any progress, so I was convinced nothing would work.
As far as treating the teething pain, we had some success with this gel. One of the best purchases of my parental creer so far was this teether pop (in addition to being good for teething, it holds about 1 oz which was enough to keep my baby entertained long enough for us to eat dinner in peace!). Now we fill it with yogurt to trick her into ingesting solids.
I use an S2 with this battery pack velcroed to the base. Works great! I can get 20-25, 20 min double pump sessions on one charge.
Not really. I've pretty much always worn tanks under everything, so that wasn't really an adjustment for me, and the boob pads aren't really any thicker than a normal padded bra.

I'm a bit on the larger size and I had pretty good luck getting soft cup nursing bras on Amazon. These aren't the exact ones I have, but they're pretty similar (I bought them on my regional amazon, the link here is for the U.S.) amazon.
Also, I found great nursing tanks in larger sizes at H&M and really cute tops, and even a nice dress that I've now worn to a couple of weddings at PatPat (link here).
