I did a quick scan. Looks like they're measuring expired carbon monoxide (CO) levels. As best I can understand, they're asserting that if holding your breath longer increased THC absorption, then other measures (like CO) should increase too. Because the same amount of CO is expelled whether held for 0, 10, or 20 seconds, there's no point holding it in for longer because you've maximized the amount absorbed.

In Massachusetts you currently can't use medical necessity as a defense because of Commonwealth vs. Joseph T. Hutchins. In 1991 the state supreme court said, "In our view, the alleviation of the defendant's medical symptoms, the importance to the defendant of which we do not underestimate, would not clearly and significantly outweigh the potential harm to the public were we to declare that the defendant's cultivation of marihuana and its use for his medicinal purposes may not be punishable. We cannot dismiss the reasonably possible negative impact of such a judicial declaration on the enforcement of our drug laws, including but not limited to those dealing with marihuana, nor can we ignore the government's overriding interest in the regulation of such substances."
This roughly means that they think the government's ability to control pot is more important than the lives of it's citizens.

The short answer is: weed can be risky for certain people with certain brains. If you have a history of schizophrenia in your family, you might be at risk. Well, read the book, he explains it better than I can.
