Holy trainers and their knockoffs do have a small hole that goes all the way through, I suppose in theory you could find an appropriately sized nail and drill a small hole through the end that protrudes and put a padlock/plastic lock through that.

This was my favorite Amazon one. You will have to bend the end bars closer together so your head can’t poke out but that’s easy with pliers. They make different versions of this one. I'm an inch shorter and I went with the ht v3 (although now it's the v4) nano, which works well for me. Currently I'm in the cage at the below link, which is much smaller and more satisfying, but could take some getting used to if you want to wear it long term. The colander style opening on the front will also hurt the head of your penis when it sticks through, which it will. I am fairly small, and the one I have is a bit longer with that style of tip.

I've been playing with it for a while, and it can be whatever you and your husband want it to be. There is no right or wrong way to play.
Personally, I love the idea of being locked from the perspective of teasing and denial, bondage, and power exchange. The more extreme fantasies like permanent locks, cuckoldry, forced bi or forced fem are actually a turn-off for me.
It took me a long time to find a device that is comfortable and fits right, so be patient and understand that there will be some trial and error. Look at it as a journey.

It is kind of hit or miss on what cage is best for any one person. The cages on Amazon are cheap so you can try a few and get an idea of what cage you like best. Once you know your ring size you can try some metal cages as well.
Need some sort of container you can seal a key in with a tie. I like a small length of pvc, longer than the key, that the key fits inside, drill a radial hole all the way through, big enough for the serialized tag to fit through, put key in, pass the tag through the hole you made, going through the key's keyring hole, so that the tag is holding the key inside the pvc pipe. Just break the tag if you need the key.

I usually recommend people try a Holy Trainer clone. The only real downside of the clones is you will want some sand paper to clean up some of the sharp edges. The upside is they are cheap and you can find out what size rings, etc works for you without spending a bundle. If you end up enjoying it the real ones I think are worth the price. For more strenuous things like running I use a jock strap. The only thing I've had much problem with is certain lifts that tend to stretch that area.
The cage style can make all the difference. I tried exercising with the tube one on once and it was pretty uncomfortable. I've had much better luck with the more open metal designs. Alright...you're a bit...bigger than most. It depends if you want a tube type like the one above, or a cage one like This. It's probably the best I could find on amazon, size wise, and for comfort, probably. That ring looks odd with the hinge.

You helped me realize the problem is the gap between the cage and the bottom of the ring. By bending the cage downward, I've made that gap smaller so that I can't slip back through so easily. It really seems to have made the difference.

First of all, you buy something like this:
The parts start at 3mm and then stop at 18mm, which is already very large and allows the insertion of a finger!
Now you train your urethra with these Hegar dilators until you can insert 7/8 mm without problems. Then you cover with it well and gladly 99% of all dilators on the market for chastity cages (whether brand or China).
Note: It will burn, this is normal, it will go away with time and frequency.
Work sterile (even lubricant!) and drink a lot, really a lot that you have to pee often afterwards. This flushes the urethra free again and prevents infections.

It is one of the better guides and books out there to discuss this with your wife. The first issue I had with long term wear when using a non-elastic strap was the strap would dig into the sides of my hips.
