It's a DOS version of the game running off DosBox and having a lot of limitations on modern systems.
It is HIGHLY recommended to use CorsixTH (think "OpenTTD project" for Theme Hospital) with this release. It just hit v0.40 recently and the game is fully playable with it. They are just adding additional features.

CorsixTH basically this is Open Source remake of Theme Hospital (which require the original theme hospital data) - similar to OpenTTD.

Another classic I see, spent many many hours in the demo alone.
I would suggest getting CorsixTH, it updates the game and makes improvements as well as fixing bugs and making it easier to play on a modern system.
I was just in a TH playing binge a few months back. Had to go through some rigmarole to play it since it's DOS only, but you can do so in a couple of ways:
1) Use a DOS emulator to play the original DOS-based game (a bit unwieldy)
2) Buy Theme Hospital from Origin or GOG.com (which includes a DOS launcher and renders the limitation of Theme Hospital being DOS only completely transparent to the user)
3) Use the CorsixTH rebuild of TH (which requires the original full game or demo and then deploy the CorsixTH overlay). Looks like there was just a fairly big release 20 days ago too.
The CorsixTH open source rebuild is the best way to play this. For one, you can do so for free and secondly, this build has fixes for resolution so you don't have see a stretched or black boxes on the left/right and there's no CD detection that occurs so no need to work around that. The rebuild has the same old story mode or whatever it's called and some new features in it too like building your scenario etc.
Edit ~ You can use the demo as the basis for the CorsixTH Theme Hospital rebuild hence "it's entirely free (and legal)" to play it in this way using the CorsixTH open source version of TH:

For those wanting to play in HD with Theme Hospital Open Source (you need to own the game, so thanks Origin!):
Step 1: Go to Origin Games/Theme Hospital/data/ and copy "Game" folder into a new location.

I am a huge fan of this game, but the problem with this release is that it is a shitty DOS version.
However, there is an open source project called Corsix-th which modernizes the game, adds support for modern resolutions, revamps the menu, adds support for custom maps and scenarios, and fixes a crap ton of bugs and quirks in the original game. It is essential if you want the best possible Theme Hospital experience.

I was having a look at CorsixTH on android the other day and stopped when I realised you needed to own Theme Hospital already. Guess this solves that problem!
I see the walkthrough at the bottom talking about compiled/uncompiled versions, and I'm not totally familiar with Github. If I "Download Zip" on the right of the file list, will all work as described in the readme?

Theme Hospital was one of my absolute favourites as a kid, can't go wrong for $1.79. The CorsixTH mod adds a ton of modern improvements to the original game without really losing anything.
Sadly, the game isn't distributed by EA in some countries. It was the same thing for Sim City 2000 in France. But hey, you might have Peggle for free, again, instead.
If you really want to play Theme Hospital, I would suggest checking CorsixTH. You can use the files from the demo and still have a better experience.
Almost. The core graphics engine is still written in c++ for performance, but all of the game logic is written in python. CorsixTH is a nice little open source example of using a scripting language (lua) alongside a core of c++ for performance. I actually just read that even Bjarne said that c++ was always meant to be used with a scripting language to simplify the workflow.
I just remove the receptionist desk when the inspector comes, and then it bugs out and you never get penalised or any further epidemic.
Buuuuut if you don't want to cheat, there's a useful guide here. Epidemics are essentially random (a nasty event like earthquakes) so no there's no means of limiting them. One you get one, the best plan is to attempt a cover up, and pick up and drop nurses on the infected to get them cleared. Extra nurses on staff will help.
Check out CorsixTH, an 'open-source' (requires original game files) rebuild and freshen up of the game.
Still some work to do on it, but it's pretty much all there for a quick game or two
You can still buy it from origin. I downloaded it when it was free a while back. If the free version of the game is anything like the one you'll buy, you'll have no problems! I didn't have any problems throughout the game.
I tried playing it with CorsixTH (it gives better resolutions, you can zoom in and out and other little things that makes the game more convenient [like when you buy some items for a specific room, it remembers the items used when you build the same room again]).
However, I had a LOT of bugs/crashes/freeze while using it. Others on the other hand says they've had no problems. Maybe try that out?

I love Theme Hospital, Theme Park, Jade Empire and SimCity 2000.
Licensing rights can be tough.
Unfortunately, stock iOS is literally the only operating system in the last twenty years you can't get Theme Hospital for. There's an open-source remake called CorsixTH that, with the appropriate data-files, is a somewhat accurate reconstruction of it. It runs on Windows, Mac, Linux, and even Android. But due to Apple's somewhat draconian app-store policies, you can't get it on iOS unless you're jailbroken.
Maybe not even then - there isn't a lot of impetus towards providing an iOS build.
So, if you get a device that isn't iOS, look up CorsixTH and have fun!

Another suggestion is to start by taking a look of the source code of some open source tycoon games.
You can also look at the source code for games that aren't necessarily tycoon games, but have elements in them that you could use for your game. The game interface, the way the game is rendered (isometric, 3d, 2d, etc.) and take your inspiration from there. See if they're implementing libraries that you can use. Depending on the license their code uses, it may be permitted to literally re-use their code.
Not really 100% free because it requires files from the original game (which you can grab from GOG nice and cheap), but there's CorsixTH, an open source re-implementation of Theme Hospital
Dune 1 was brilliant! As for Theme hospital: it's on GOG right now and there's an open source clone at https://github.com/CorsixTH/CorsixTH which works quite well. I used to build rides in theme park with sharp corners etc so people 'd get sick on purpose - kid sister loved to see everyone puke all over the park ^^
It seems like it's still pretty active on its github. You can download the latest dev build as well.
Not sure about ELI5, but say you want to install CorsixTH after fixing a typo, but they only provide a pre-compiled binary for Windows, so you download a copy of their source and fix the typo. Github says it's mainly written in Lua, C++ with some Java and C and you read the How-to-compile page to see what you need to install - cmake, g++, sdl, lua (just for the required libs). So you go ahead, lookup the debian names (cmake, g++, libsdl2-dev, liblua5.2-dev), and install these libraries, cd to the correct directories and run cmake and make. Say you get a successful compilation, then you still get error messages when trying to run it, because you haven't yet installed lua-filesystem and lua-lpeg and even then, the game will run entirely silently because you didn't have any of the optional dependencies for the sound-related features. You could write a blog post on your experience, or write an "on Debian" section of their wiki page, or even write a script to automate it, all of which is specific to that one piece of software on that one distribution.
Or, you notice it's available in the repos, so you just run the non-software-specific:

In short:

The servers are fine. All what is missing are more keys. Due to the high demand, EA didn't have any keys left. They have to generate more, so that they can offer RA2 again!

Someone remade it. You still need the original files (which Origin has for sale, if you want them), but this remake gives you bug fixes and proper optimisation for new systems, as well as high resolution, zooming and a few other things.

Not as funny and a pure sandbox (no missions, objectives etc.) you should take a look at Banished.
Theme Hospital is a really great game. I used to play a lot of it on psx and replayed it recently on PC. The game is a bit old, but, good news, a team is working on adapting it for newer hardware and correcting the bugs. Check CorsixTH. The best part is you only need the textures of the game to use it, you can find them in the free demo. You will find all the info in the wiki of the project.
You could also take a look at other older games like simcity 4, zoo tycoon franchise or the theme park franchise.
Theme Hospital is regularly $2 on GOG, and there's a fan-made update called CorsixTH that adds support for modern PCs.

freeablo has been in gaming news a bit recently. It's a reimplementation of Diablo. Still lots to be done there too, but it's a really impressive undertaking.

Also, you can use either version with CorsixTH. It's being recommended in every thread I've seen about this offer, and for good reason. It's an open-source remake of Theme Hospital that requires a copy of the game's content.
Just because something 'uses all the files' doesn't mean it's 100% there.
Ten to one it's using the art and music assets from the TH files rather than any of the code, that tend to have a different 'copyright' to them than the source code of a game does-- this why, for example, when the Doom Source code was released, that didn't mean the main iWAD, that contained all the graphics and levels and music, was, just the engine itself. (Which is why a project like 'Freedoom' exists. To provide a free replacement for that which wasn't open source.)
Aswell, CorsixTH is not a port, it's a clone. The main site admits this: https://github.com/CorsixTH/CorsixTH --- so truthseeker_x is right in his assessment.
The engine shouldn't be confused with the assets of a game. and Ten to one the engine is still missing pieces if it's in active development.

Sorry to be a downer but I've never seen a fan built remake of any game completed ever. The only exceptions are Theme Hospital (CorsixTH) and Dungeon Keeper (War for the Overworld). The second was definitely not cheap or a small project what so ever.
It happens fairly often that fans of a game which has had a disappointing presence in the market try to remake it themselves. It never works out.
This is too ambitious. You can do it anyway as a hobby but don't expect the game to get done. You're going to want money for it if it does as this is a serious investment of your resources.
When I did this, I didn't use the demo so I can't say for sure, but it looks it is given the wording from this link (which would make sense, however you could still use the other features the CorsixTH gives you like building your own scenario, etc):

If you're looking to play Theme Hospital again, check out CorsixTH. It's an open source clone of Theme Hospital with some improvements. I haven't tried it in awhile, but it was fantastic fun, although some features were missing then, looks like it's gotten better. It requires a gog.com version (or the original CD) to play.
