An actually privacy friendly qr scanner is this one developed by the University of Darmstadt, Germany. It doesn't ask for Internet connection so it can't spy on you by definition.
Ha, I actually was just looking for one tonight while reading a bit in bed. The one I ended up with was screen filter. I'm assuming you are only wanting it dimmed, not tinted like the Twilight app. No permissions, and I haven't noticed any ads even. I don't think it's too hard to find one without permissions though if you want to look at others.
If you like simplicity and don't mind the lack of widgets, you could use Hayai Launcher: it's open source and doesn't require any special permission. And is extremely lightweight.

Just a note, though- before stringing up an app for abusive permissions, please remember that Android's permission system is inexcusably terrible. Google has ignored the problem for years, enabling many scumbag devs to abuse the system. Pleas for an opt-in system like the one used by iOS or a tool like AppOps continue to be ignored.
So that said, please check the app description to see if the developer has tried to explain why the permissions are needed- sometimes it appears much worse than it actually is. Often you'll have dissect an app to see how a permission is used- I'd be happy to help with such an analysis occasionally.
Weawow does that. I just found that app last week, when I searched for an app which displays the temperature as a continuous notification on the lock screen.
I really like aCalendar+. (There is also a free ad-based version) It syncs with multiple google calendar accounts, lets you color code your events, set multiple alarms on any event, allows flexible parameters for recurring events, lets you choose what hours it shows on your daily view (handy for those of us who don't work 9-5), and includes widgets for daily, weekly, monthly, or running agenda views. It's very flexible and very customizable.
Permissions:
Modify/View Contacts (Used in the location feature, which will allow you to add a location directly from your contacts, then get directions from Google Maps, as well as for sending invites to people for meetings.)
Add/Modify/Read calendar events (of course)
Find/Use Accounts (for syncing with Google Calendar)
Control NFC (I believe you can pass calendar events to others via NFC. I've not tried it, though)
Full Network Access (I assume this is needed for syncing and sending/receiving invites, but I'm not 100% sure)
Run at Startup (So it can give you alarms/reminders.)
Control Vibration (For alarms/reminders)
Prevent Phone From Sleeping (so it doesn't sleep through alarms/reminders)
Read Sync Settings / Toggle Sync (so it can sync with Google Calendars)
And it warns you if the app includes non-free software or libraries (free as in speech), or includes advertising, or promotes non-free extensions.

Of course I can. I have the source and I can make the modifications needed. Removing contact discovery would probably be the hardest. Removing functionality is easier than adding one, and finding whatever pieces of code that are responsible for sending/recieving photos (the modify files on device) and modifying contact discovery is well within my reach, even in Java.

I know what you mean about reasonable permissions. It's hard to know the intent of the app/designer/company.
If you are interested still in removing the blue tint, bluelight filter doesn't require location like twilight does. Although I believe that the location on twilight is just to figure out sunrise/sunset. blue light screen filter doesn't require as many permissions either, but it has in app purchases. I've never personally used either, so maybe someone else can give you other recommendations.

Oops! App lock is the one I use. Not a lot of permissions. I don't remember ads, but I ended up buying the pro version anyway after trying it out. I do remember it didn't work for a short while on lollipop but I think it was fixed pretty quickly.

Vanilla Music is good enough for me, most of the time. It could use some better tag-support and searching, but it's free and open source!
I'm not quite sure what you're asking. I don't use the app myself. I'm guessing when you search a QR code, it uses your phone's default search engine.

You only need to set it up once, and Adguard DNS works with any wifi network, so it should be fine as long as your kid doesn't figure out how to switch it back. It's less obvious than something like Blockada, and changing the DNS doesn't seem to drain the battery as far as I can tell.

What is "the other" QR scanner mentioned in title and is 10x bigger? Compared to Barcode Scanner, Privacy friendly QR scanner is twice as big and has less features.
Barcode Scanner do require a lot more (legitimate) permissions though and I think by default it is set to lookup barcodes (not qr) online.
Edit: The other one seems to be QR Droid Private™, I've just seen the post where OP commented.

It is very simple the way it looks and functions. It is Holo Dark themed, which is really it's only fault. But at night and early morning, I prefer dark apps anyway. Free version is not limited in any way. It just has an ad, which can be removed for less than $3.
I don't understand why you say we don't have App Ops. Maybe your title/post is misleading and you want it without root?
