I had a similar problem, except that I didn't get mentally drained out as fast.
(as for the motivational part) I played it like a game. I used lifeRPG (a mobile app). I made myself achievements. I created a little ratio system.

I guess the huge reason that worked is that I was just so sick of myself of playing games and surfing reddit/imgur all day.
EDIT: I would recommend to just keep going at it. When I first started this (2 months ago), I couldn't study for more than 4 hours straight. Now, I could just study the entire day and barely feel exhausted.

As an Eagle Scout, I fully support this and would buy at least two.
Make sure it has a scientific calculator, a unit convertor, clock, calendar, stopwatch, timer, and notepad for sure. Maybe a mobile observatory, and LifeRPG.
I love LifeRPG, it would help keep me organized so well with a lot of things post-societal collapse.
Seen alot of people suggest Habitica, didn't stick for me since I felt like I was improving my characters life ingame and not my own. I like this one because its a very clean UI, very simple, with a nice honor system that keeps you in check, and only a slight penalty if you fail a mission. Definitely improved my quality of life so far
Some psych research suggests it takes about 30 days to build a habit. Aim for a good month, and hopefully things will become easier from there.
Something else to try is gamification - incorporating play and rewards into your goals. You can do it in your own way, or there are also things like apps which combine the idea of a to do list with an RPG, e.g..
I haven't tried Microsoft To-Do, but I've been using Google Keep.
The main thing I use to motivate myself is LifeRPG, your very own (customized) RPG life.
You set your own tasks and get rewarded a certain amount of points which you can use to redeem a reward. ^(You set the points and costs yourself too.)
There are also skill levels where you may or may not decide to give bonuses if you reach a certain level/tier. ^(I personally give discount% to rewards. Yes, tedious...but it helps me think I should prioritize those.)
Highly prone to cheating, so you can train honesty at the same time :)

It is a bit more cumbersome to learn at first than habitica, but it works offline. I think the biggest downside for you is that there’s no art style at all - all the rewards are text-based stat increases or custom text-based rewards. It feels a lot like one of those very old school RPGs.
It may not work for you because of the last point, but I guess it’s worth it to put it out there.

If you don't like taking screenshots of nasty looking notes and making it your lock screen, I would suggest looking into this app called Life RPG, where you basically turn your life into an RPG and Grant rewards/XP/milestones, track your XP/levels and missions, and set reminders and repeated events. This is pretty handy to have for this that like to have some fun in life and make it a little bit more bearable.

I've devised a reward system for myself. 4 hours of study = 1RP (reward point), work out 1 hour = 1 RP, long term project finish = 5RP, etc. Rewards points are spent on, you guessed it: rewards. Order out cost: 1 RP, a reasonable personal purchase cost: 1 RP. So on and so forth. I don't really feel guilty about enjoying my rewards because I know i've truly worked for them (according to my economy, which is always adjustable). To make things even easier, there are apps that literally track these things for you too. Check out Life RPG. The XP system is a literal bonus for you achievement/trophy hunters but it applies to YOU directly. My basic theory is I won't get to enjoy much if I put no work in. The discipline comes in where you hold yourself accountable to your plan. Like Shia said with his nike shoes on: Just DO it~!

I like using the new Life RPG app, it's not like any usual to.do app (former any.do user).
It would help to know what OSes you're dealing with.
If you have Android, LifeRPG is a popular one, free, that also has a subreddit though not, I admit, a terribly active one.
Myself, I hacked together motivational tools from un-gamified sources. For example, I'm having great success with Rewire, which (if you switch to percentage tracking) lets you create multiple progress bars for various goals.
But I know you're looking for a ready-built solution. Bizarrely, though, although it isn't a game per se, Pomodoro Challenge is probably the best one of these. There's a pro version with more features, but basically, it's a well designed project tracker that uses Pomodoro intervals (Ironman style - i.e., no pausing) to unlock personal achievements. You start it as an "Unrepentant Slacker" and work your way up the commie tree (it has an ironic-ish Red Communist theme). The writing is pretty entertaining, which really sets it apart.
