Here is the EasyPeasy book, its helped a lot of brothers and with its help, I've been completely P clean Alhamdulilah. In general I would advise against changing your routine too much, because that might make it harder in the long run, but deleting or restricting your Instagram/social media definitely helps. You can also try this blocker for your internet, to restrict your access to sites. May Allah make your journey easy!

Motivation will die down. It's up to you to implement discipline. If motivation is your launch factor - the first 10M in a 100M sprint, discipline will carry through the other 90M.
You also need to develop a system, rather than chase a goal. Chasing goals is good, but developing a system to achieve that goal is even better. Try and develop an adaptable system that allows you to stay away from PMO.
Lastly, being optimistic is encouraged in Islam. Optimism can be viewed as trust in Allah - you trust Allah to do whatever is best for you, though you may not realise it yet. Yes, PMO is a nasty habit, but you should view this as a challenge to grow. Never give up hope. Once you give up hope, you've given up. Period. Read Viktor Frankl's book, "Man's Search for Meaning". One of the key tenets of this book is that hope can and will carry you through the darkest of times (it gives the example of people stuck in the concentration camps during WW2 - even when they had nothing at all, they were still able to pull through - they held on to something they truly loved and believed they would see that thing again, no matter what).
You can do it! Have patience, try your best, and it will happen.
Wa alaikumusalm brother. I was addicted to pmo for more than a decade and this is the longest streak I ever had after so many small streaks. From my experience I can say "A habit cannot be eradicated instead it should be replaced" ( a quote from the book The Power of Habit by Charles Duhigg, if you have time you should read it, it will give you real idea how habits controlling us). Replace your bad habit with something good, like exercise, Quran, reading beneficial books etc.
Don't ever think you cannot do it, its the biggest trap of shaithan. When you relapse get up make tauba and start again, this is a long journey.

You might get sick of hearing this, but you're young and if you fight it now it's a slightly easier battle. Personally I had PMO issues for just under a decade.
As far as advice goes the most effective method is fasting. Especially because you're at a age where you're testosterone is being produced rapidly. (It is more effective in summer imo)
Work out. You're young so don't do weights and bench presses. I'd suggest calisthenics. You can buy a pull up bar (no screws) (there's obviously other ones you can buy). If you're new to it start with a routine of pull ups and push ups. The reason I suggest this is your at a age where you body will find a way to burn energy one way or another, Even if you're not "athletic".
Hold on to your Salah. The spiritual benefits I am not able to summarize, but from a habitual point of view it creates a routine. And before you pray you can't be in a state of major impurity. Hence there'll be times where you thinking of PMO but you might not because you'll think "ah I'll need to do ghusal anyway because it's magrib soon" which will deter you from PMO.
May Allah (swt) make this a test easy for you. insha'Allah the more you fight against it the more you'll be rewarded.
If you want to do something productive, go to coursera.org. There are lots of free courses on many different subjects, by international experts.
If you need human interaction, you will find groups with your interests on meetup.com, what ever those interests may be.
As for gaming... I agree that it's an activity that leads to a lot of solitude and disconnects one from people. I've replaced it with occasional board gaming evenings with friends. There's a huge plethora of game types out there with are much more social and stimulating that sitting alone in front of a computer for hours on end. I'm not a fan on Dungeons and Dragons, but if you enjoy RPGs and have some friends who do, that may be a fun alternative social activity.
It's better to be in good company than to be alone. It's better to be alone than in bad company.
I recommend you recite Ayatul Kursi before and while you're in bed and make Tasbeeh until you fall asleep.
Practical tips:
1- Start going to sleep on time consistently
2- Stop using all electronic devices at least 30-60 minutes before going to sleep. If you really need to use your phone/laptop, I recommend you install f.lux on your laptop which minimizes the emission of blue light which supresses the release of melatonin; the hormone that makes you feel sleepy.
3- Before the lockdown and quarantine, I would go to the gym and get a late night workout in. This helped get me out of the house and helped keep me in shape.
As an alternative (if you don't have access to a gym or can't leave your house) I recommend walking around the house, doing some push-ups, and taking a shower.
Do you get urges from porn in general or is it a specific website you visit. If the latter, try installing a website blocker to prevent yourself from getting these thoughts. If on pc try cold turkey. May Allah SWT make it easy for you.

What helped me stay away from my urges was installing a website blocker. I use this one, and it can "essentially" permanently block any haram websites. Give it a go. May Allah SWT end our sinful habits. Ameen.

Download it and add all the haram websites you visit when your urges overwhelm you, and then set the time duration to "as long as possible" which blocks those websites for 16 years. The best tip I can give is to spend time on something else. There must be some new skills you want to pickup, or a subject you want to study.

I also picked up some low-level editing skills to make some Islamic videos, just to fill free time. Now I'm hoping to spend the next few days making a simple little game with the Godot Engine.
Spend time remembering Allah SWT and distract yourself from the haram.
You can have a barbeque for like-minded people like I once suggested brothers have a "Muslim convert barbecue" and it turned out to be really nice. You can also check out meetup.com and join the hiking groups, wild plant, wild mushrooms walks, things that have no fitnah and bring you to ponder over the creation of Allah. Also, participate in everything the masjid does like a cleanup day, classes and prayers. These are all easy ways to meet people.
If you live in a big city you can start a Muslim group on meetup.com for brothers to get together and ponder over how to benefit from ayaats from the Quran as you all take turns reading it English. You can also start a boxing class or tea drinking time between Maghrib and Ishah in the Masjid and lots of other stuff. If you don't ask the answer is always no.
As you make more connections remember that contentment is not something to search for it's a decision to not need the dunya so much. It is definitely a good idea to make more connections and remember it takes time to build quality connections so don't force it, just do it consistently and naturally.
