IBS is a catch-all diagnosis for problematic bowel symptoms of unknown cause. It’s an idiopathic disease, meaning that they don’t know the origin.
A diagnosis of IBS stays unless you’re randomly cured or they identify the actual underlying illness. There could be a plethora of causes of IBS.
I personally use one of these to keep in my work bag. I carry around various tablets so it works well for me.

Your comment about Imodium being an opioid caught my eye. I had to look it up because I use Imodium often -- usually one at night before bedtime and another in the morning.
Apparently, heroin addicts use Imodium to get high if they can't get their hands on heroin. Here's what the New York Times had to say in an article on that topic:

SIBO/SIFO/SIYO are often mistaken/linked with IBS and are worth checking into.

I think this is something worth looking into.
Miralax always makes me feel absolutely terrible. Anything that simply softens the stool, I've found, does nothing to me and just makes me feel bloated and nasty like you described. My problem is more to do with my body forgetting to push things along, so I've found that I always need a stimulant element involved somewhere in there.
I strongly recommend trying triphala. (The brand I linked is one I've found success with! I take two tablets daily when I feel the need to). Someone recommended it to me at a local pharmacy a few years back and it's my favorite remedy by far. Careful not to overuse it though or your body will just kinda get used to it and it'll lose its effectiveness. It's worked wonderfully for me though; never experienced any pain or discomfort, and generally works within 48 hours.
I also get backed up if I consume too much fiber for the most part, but this cereal worked like magic for me. It has both soluble and insoluble fiber which is probably why? It's expensive so I've only bought it a few times but it absolutely worked as advertised!
I was always wary of trying new probiotics because every time I tried them I'd get really bad diarrhea for the first few days and I just didn't like that. But a few months ago I was really sick for awhile and my mom suggested these probiotics and they really helped. I've been taking them ever since and I haven't had one episode of diarrhea and that awful sickness went away. I still have my usual IBS symptoms, but they're not as severe. Though I will warn you they do make me a bit constipated (but I've always struggled with constipation), but I've just been eating more fiber and that helps.
Oh my. This is a great example of the link between the mind/brain and the gut. Essentially what has happened is that your body has become conditioned to have a bowel movement when you think about going out. You might have heard of Pavlov's dogs that learned to salivate every time they heard the bell being rung that signaled that dinner was on the way? The digestive system is VERY responsive to learning and experience. It's the same reason that once you've gotten nauseous or vomited after eating a certain food or drink, the taste/smell of that food/drink is forever somewhat nauseating.
I do NOT suggest pre-loading with lots of anti-diarrhea medications. The problem with doing that is two fold. First, it can easily set up a cycle of alternative diarrhea and constipation. If you end up straining with the constipation, you can stretch the walls of the descending colon and give yourself itchy painful hemorrhoids that are likely to bleed. Second, it does nothing to address the underlying conditioned response. In fact, it really just further convinces you that you can't risk leaving the house without taking medication.
The solution is to start to train your gut to respond differently to the prospect of leaving the house and to break the cycle of pain, urgency, frustration, anxiety, arousal and more pain. The best way to do this is with cognitive behavioral therapy interventions for IBS. By changing the way you approach and think about these situations, you can actually reduce the onslaught of physical GI symptoms for pretty straightforward biological reasons. There are several good CBT for IBS self-help books out there. One is called Controlling IBS the Drug Free Way. An even better one is Reclaim Your Life from IBS. You can maybe try powdered sencha. It's basically the same idea as matcha but the tea leaves have been decaffeinated using a water based process before being grounded into a powder. It might actually be possible to make it yourself by buying decaffeinated sencha tea leaves and grinding them into a very fine powder but I haven't tried it myself. I'm glad the diet change helped. I forgot about supplement stuff, my bad. For gas/bloating, I use simethicone chewable tablets (personal preference because they taste good lol, there's all kinds of forms of it) and lemon balm (make sure it's Melissa officinales) which you can get a tea or pill form. I like the tea because it smells and tastes good. Lemon balm is supposed to calm the whole GI system.
The indigestion it gives me is crazy if i don't have a 100% empty stomach. I just take it at off hours, like during a night time bathroom break, or right as i wake up.
IBGard might work better for you; its not liquid pepperment oil, but its pricey. I use Heather's Tummy Tamers personally.

Get your head straight make a plan, get you meals done by yourself control what you eat, try the fodmap diet. There is subs like r/MealPrepSunday/. Get probiotics that help your type of IBS either D or C, there is plenty of research article out there. Try garlic if it doesn't hurt you. Get plenty of sleep.

The amount you mention blood makes me worry. That's usually indicative of other issues. Ensure you have the proper tests done to rule out other issues. You could even look into 23andme.com for insight into some other issues.

It indeed has also insoluble fiber, specifically 1gr in a tbsp serving. I don’t think this quantity would affect you but the only way to know for sure is to maybe give it a try (nothing to lose) and if it upsets your stomach give it up.

I use this activated charcoal drink when I'm feeling sick. It doesn't really give me any side effects just makes my tummy feel better and I have more of an appetite. But that particular drink also has potent ginger, lemon juice, and coconut water in it. I've never tried an actual activated charcoal capsule so you'd have to do your research on that, but they can cause constipation and must be taken away from medication and food. They can also help with gas pains. I use the bentonite clay much more often than the activated charcoal drink because the drink is hard to find. The bentonite clay works similar to activated charcoal as it too removes toxins from the body. I've never noticed side effects with this product either though it can also cause constipation. I usually only take it for a few days until I feel better.

I've found that, for me, Superior Source sublingual vitamin B12 supplement increases appetite and helps food to digest more quickly while also increasing energy level.
Buffered vitamin C in the form of calcium ascorbate is the only supplement that I take every day. It is non-acidic, stomach-friendly, and may have an anti-inflammatory effect.
Each person is different but I must say that I have extra-sensitive digestion and so, in general, I struggle with finding supplements that I tolerate due to my sensitivities to the fillers or preservatives that most of them contain. Whether it is cellulose in traditional vitamins, sugar alcohols (xylitol, etc.) in chewables, pectin (in gummy vitamins), or citric acid (in liquid vitamins), one ingredient or another almost inevitably troubles my digestion too much to be worth it. Sometimes I try the pure powder supplements that are now available, but even these promising single-ingredient supplements may bother me for unknown reasons. I guess my body just wants to get vitamins from foods not from supplements. So, if you do have trouble with supplements, keep in mind that there are nutrient-rich food sources of vitamins and minerals that you could consider. If you can eat eggs, for example - those have a lot of nutrition including B vitamins and iron.
Ever since I've been on lowfodmap and stuck to free from foods aswell I've literally had no massive issues been alot better I changed my eating/drinking habits aswell. Only drink water or lactofree milk it's boring but it's helped.

Desperate like so many others on here, I got creative and decided to try something different. Hence, I bought some digestive enzymes on Amazon (link below) and they worked really well to combat the bloating!! The directions say to take 1 capsule before a meal. However, in my case, one capsule wasn't helpful so I read on the comments that many users recommend taking two instead. I actually take three! It is rough on my stomach (adds to my diarrhea) unless I mix it with a large glass of very warm water for 10-15 minutes before drinking.
I've been doing CBT for nearly three months now and recently started EMDR therapy as well. While both methods are helping my anxiety and depression tremendously, which in turn are improving my IBS, what has helped me even more is being able to live in the present moment and truly being able to fulfill the old cliche' and "let the little things go." Of course, that is the aim of CBT. I was given a book by my therapist called, "Addicted to the Monkey Mind" by J.F. Benoist. It's only $10 on Amazon and a pretty quick read. If you are considering CBT, it's very worthwhile to read. The basic idea is that it helps you be able to differentiate between things that are actually a threat and things that are not, as well as to identify the core beliefs causing issues that someone interested in CBT would want to target. I personally don't use Metamucil because of the added sugar. I don't like artificial sweeteners so I avoid the sugar free Metamucil too. I stick to plain Psyllium Husk seed powder with nothing added to it. Here is a link to a good brand on Amazon.
The great thing is that you can also use Psyllium Husk in baking if your into that.
I have more IBS-D than -C. I have been taking this for about 10 days now and I am happy to report that my gut is much happier (as measured by less gas pain, urgency, times per day). I had a thought at one point, “I am actually feeling normal again.” I am still avoiding all dairy and eating a pretty bland, restricted diet. I recently created a separate spreadsheet so I could figure out how much Imodium I take daily and where my poos fall on the Bristol scale.
This is the exact one I ordered but 32 oz is obviously overkill. I'd try a smaller amount and see how it takes to your body, should be at least a little beneficial for everyone.
Switch to Roasted Dandelion Root tea. It still cleans you out, but much much muuuch less than coffee. It tastes like coffee and it's a super food. One of the healthiest things you can consume.
It tastes like a light coffee with a touch of peanut butter. Make it how you make your coffee. So if you add creamer to coffee, add creamer to the tea.

The advice "stop thinking about it" is actually correct but it doesn't mention how to do it. Well, you have to practice "not thinking about it". You have to train your brain just like a muscle. I can recommend you 2 things:
- IBS Audio 100 by Michael Mahoney (100 days of hypnotherapy, but might need repeating 2-3 times to see improvement according to what people say about it on the internet, so actually it can take you even up to a year), I'm at week 3 now, I sleep better, my thinking patterns are certainly more positive
- "Full Catastrophe Living" book by Jon Kabat-Zinn, it's 600 pages long, 6 parts, you read the first one and start the program (8 weeks, 45 minutes of meditation every day) that is really promising from what he says in the book (you can read about him on the wikipedia, he has a huge experience and his stress clinic is sometimes last resort for many chronically ill people).
Choose one and practice it EVERY DAY. Like I said. Repeating it only once in some time won't work. You have to actually rewire neuron connections in your brain. It takes time and some energy (although both methods are really relaxing). Put your well-being on the pedestal and find time every day for your practice. I'm sure you will see progress after some time. Good luck :)

In regards to your libido, that makes total sense. I recommend this book to just about everyone in my life; it's seriously one of the best things I've ever read. I don't presume to know much about your relationship, but the concept of "context" in that book might help you explain things to your husband.
Regardless, I hope you find relief. One day perhaps medicine will understand the microbiome enough to bring us all back to a place of health. If you're into hugs, I offer the internet version of them.
I drink Heather's Fennel Tummy Tea all the time! No placebo, it really helps. The taste is not good. BUT it does a really fantastic job of reducing bloating and puts a stop to my intestinal spasms pretty quickly. I actually kind of like the taste now because I know it will make me feel better. Can't do peppermint due to acid reflux.
I ordered this on Amazon about a year ago and am really glad I did. The cookbook is nice but trigger foods differ from person to person so there are a few things I can't eat in there. If you get acid reflux really easily, DON'T FUCKING TAKE THE PEPPERMINT PILLS. They will make your esophagus cry. Also included was a handy dandy pamphlet that lists generally safe foods for IBS sufferers.
If you are looking for cheap alternative, how about instead of rifaximin something like garlic or oregano oil, and instead of the elemental diet try something like juicing.

Antidepressants, when taken long-term, can increase a person's risk of significant side effects. Please see this excellent book.
If the antidepressant is helping you, feel free to keep taking it. If it's not helping, please strongly consider discontinuing it. You might want to first ask a doctor or pharmacist whether cold-turkey is fine or whether a gradual taper is better.

I have the same problem …. It’s horrible .. I know . I’ve been doing daily or every other day enemas for years now. I hate it .. but it’s the only way . A few things that I’ve been trying lately that have been helping are :
1) sea salt in my water . You should read this book , it very informative on how much salt water. Is for your body . There is a lack of information wth regards to SIBO and FMT but I found this study which shows FMT did help in this scenario - FMT/SIBO Study
I have my suspicions that if probiotics help you, then an FMT will be beneficial while those that experience an increase in symptoms are likely going to get worse. I have SIBO and take probiotics which actually keep the majority of my symptoms at bay...only one I have is gas which I can live with as before I was running to the toilet with constant diarrhoea

Got this one back at the first hint of TP shortage. I now go through about a roll a month just to dry off with a few sheets. Of course, women will want a model that can wash in a more appropriate direction, but since I live alone now, there isn't much female traffic in my bathroom...and there's still TP for them when they do use it.

Kang Ning Wan Chinese medecine herbs, takeling them after you've eaten something that doesn't sit well. Seems to reduce the inflammation and discomfort for me. Upon my acupuncturist revommwndation. Peppermint oil and also ginger pills are helpful for me when I'm flaring up. Did doctor consider hidradenitis suppurativa? This can hurt and drain like crazy. HS is definitely related to IBS. Not a lot can be done about it, unfortunately.

It’s more discrete so u can also hide it if you live with someone or have ppl over. There are 2 with 500 ml capacity. There are electric versions available with higher water pressure but they cost about $40 each and have a lower water capacity too but if you prefer that I can send that link too. Also I get your concern about the lota it’s advantage is that it’s made with a longer nozzle so you can reach and the opening is larger too so water flows rapidly, so it’s made for this purpose but obviously you’d use toilet paper with it too then.

My favorite physical book recommendation is The Low-FODMAP Diet: Step by Step by Kate Scarlata & Dédé Wilson (runs ~$21 on Amazon). Clear lists of food, example menus, and very easy to make recipes.

I cut out gluten anyway to see if it's like a wheat allergy or something, but I'm not very good at always checking to make sure that things don't have it hidden in it! For example many brands of soy sauce do.
It could be stacking, I'll have to be careful. I also cut out sparkling water incase that is somehow causing issues (gas, acidity).
Recently I started drinking sugar free coconut milk kefir. I've noticed that my symptoms have improved. Before I cut out sugar I drank kombucha and ate plain skyr yoghurt daily - both probiotic. So I'm wondering if somehow I need to find some really decent probiotics. I had to stop the skyr yoghurt as I eventually worked out that even without lactose milk was causing nausea - as a result I don't eat any milk products at all.

It worked very well for me when had methane type SIBO (constipation) but not as well for my hydrogen type SIBO. It helped with the pain a lot and just in general tolerating foods, and feeling good when eating, as well with pooping.
It is mostly available in Southern Europe where people speak Spanish/ Italian which is why you likely cannot find reviews for it in English.
You have to buy it in Europe- as they do not ship it here (to my knowledge). So you either have to have a contact in Europe, or buy it when you go to Europe. It is not very expensive- about 30 euros for a bottle of 90 pills if my memory serves me right! I have had family in Europe send it to me before, and it made it through customs.
I found the Italian amazon site- they have 745 reviews of the product and a 4.5 star rating of the product. The reviews are of course in Italian, so it may be hard to understand if you do not speak Italian/ Spanish... I speak Spanish and can understand it some as the languages are similar.

I will note that it does not seem to help all people- but for those it does help, it is very much appreciated.
