It's an old white Pixel 2 that was just sitting in a drawer, running PC monitoring software that is super easy to setup vs. a small screen/raspberry pi method. I have a regular USB C cable that keeps it charged plugged into the back of the PC. It stays on even when my PC asleep, the gauges just freeze on the last reading.

While he didn't outright say he wrote the software it did sort of sound that way based on his post. But considering the UI on that phone I'm guessing english isn't his first language.
Yesterday I was able to get rid of the stutters and the D3D_INIT error by ramping up the fans of my card.
I changed it using msi afterburner, after it hits 50°C the fans climbs up from 40% to 100% when 60%, I also opened my case and used another fan to make it cooler. Extreme measures, but I can now play with constant fps at 1080, before I had to downgrade the resolution and the temps were at 75°C.
Also I disabled shadowplay, steam overlay, msi afterburner monitor, so I can't see my current FPS, but it looks smooth, and no stutters.
I also use Remote System Monitor, to keep track of temperatures without the need of an overlay in the game screen.
I'm really just guessing to be fair at the moment. Try running some monitoring software to give you an idea.

Nope, super easy. Used and app called Remote System Monitor. Download the pcversion and put the phone on the same wifi/network. Then you can set up the phone to launch whenever it receives power. I just ran the usb wire to the back and into an open usb port.
I had an old phone Nexus 5X sitting in the drawer, just disabled the BigCores because it had a bootloop issue. But any phone will work.

Thank you so much! The stock FTW3 just doesn't match my PC's aesthetic at all and I've never painted a GPU before so I'm happy it turned out well.

This method only took me about 30 minutes to setup, I think the raspberry pi method is much more complicated and I think also uses an HDMI slot?

Google suggested Remote System Monitor for Android. For iPhones, Pulseway might be what you're looking for. From the description:

Not sure if it can only notify, or also list temperatures though...
What worked for me is to tweak my gpu fans.
Before this I had to play at 1600 x 900 and had some stutters while driving, GPU at 75°C, now I can play at 1080, GPU at around 55°C.

However, leaving a phone inside of your pc case charging non-stop seems like a very bad idea due to heat generation, expanding battery over time etc.
I would like to know how I should approach making a couple tweaks after rooting the phone (Pixel 2 in this case).
Are these things possible? How might I go about accomplishing this?

It didn't work in Windows 10 shortly after the very first upgrade wave. I don't see a bunch of recent comments about it not working, so it might be worth a look. Maybe try it if that NZXT cam ends up not working.

I have been looking for something like that as well (repurpose old phone/tablet for displaying stats/temps etc).

I'm using this to monitor my computer while gaming. Gives you temps, GPU usage, etc.. You don't need to install anything on the computer, just launch the server.

Some people even put lcd stuff in their aquarium style pc cases, including lcd readout AiO coolers. Not really my thing as I don't keep my pc on my desk surface but whatever floats your boat.
